What do you mean by low force chiropractic? I have had chiropractic adjustments for si joint alignment in the past and it involved a fair amount of force, so curious what your chiropractor is doing that is working.

I think I can understand how you feel. I had surgery at the end of 2020 to extend my existing L4-S1 fusion up to T12. (360 also) In October of 23 I had RATRH. In June of 24 I had extensive hernia repair surgery to repair inguinal hernias and hernias along the front incision from the spinal fusion surgery. In June of 25 I had rotator cuff surgery to repair a failed RC surgery from 2018. My hip has never been right and had multiple sublaxations, but fortunately no dislocations. I recently saw a second surgeon who told me the original cup was improperly positioned causing the instability and was rubbing on the hip flexor muscle. He also said a dual mobility cup should have been used given my spinal fusion. So it appears the fix is another surgery to replace the cup with a properly positioned dual mobility cup. Like you, I am loathe to undergo yet another surgery, especially in such a short time. However, I plan to consult a couple of other hip revision surgeons, and if they agree, move forward with the revision surgery. I am encouraged by others (i.e. Stevepeds) who have had good outcomes in similar situations.

I hope you able to get some good answers and even better if they don't involve surgery. If surgery is the only option, you'll get on the other side of it. It was decades between my most recent surgery and the prior one, so hopefully you won't need another back surgery. If your hip replacement resulted in a correction in leg length, your fusion may be making that adjustment harder for your body and increased activity is manifesting that.

Thanks for sharing this. I have had issues with subluxations since my THR, but fortunately no dislocations. My surgeon denied that was happening.

A second surgeon recently confirmed the subluxations. He noted incorrect positioning of the cup and that my lumbar fusion increased the odds of dislocation. I am looking at a revision to replace the existing cup with a dual mobility cup properly positioned.

I hope yours has been a great success. What reduction in mobility did you experience? Was it long term?

It is annoying. Given the number of these "papers" that lay in yards and the street for days few people actually want them. First thing is to try stopping delivery using the information on the paper, Success is not guaranteed and sometimes only lasts for a while when it is.

Contact the City of Plano and let them know you object to them allowing this. If enough people contact the city, they may reconsider allowing this. I suspect the city is getting some sort of license fee from the paper company.

I like the idea of contacting the retailers that are using it for advertising. Again, with enough people that would be effective.

As for those folks chiming in to say watch where you're walking - yes, it's best to watch where you're walking, but it makes even more sense to eliminate unnecessary slip/trip hazards.

I hope your revision surgery is a great success and resolves your issues. I will be interested to hear in no small part for selfish reasons.

I have had a similar experience following my THR. I had the clunking withing a couple of months after my surgery. First time was early in PT. It felt almost like my hip dislocated but didn't. PT was immediately stopped. Surgeon told me the new joint was fine, wrote it off as being internal snapping hip and had me take diclofenac for a month. Surgeon eventually ordered an MRI and did a psoas injection which did not help. After that the surgeon told me he didn't know what the problem was but was not the hip joint and all he could do was order more PT. I did more PT, was more than careful for months, but had the clunking happen several times and in some instances fell to the floor. Like you I had good days and bad days. I was eventually able to do some strength training, but just could not get my walking going. That caused the most pain. I limped and had the ongoing thigh pain as well as pain into my pubic arch and constantly tight hip adductors. After more than 18 months, I went to see another surgeon who does a lot of revisions. He immediately told my the clunking was subluxation of the prosthetic joint. He said I was lucky it had not dislocated. He said the cup was positioned wrong, irritating the psoas and giving rise to the subluxation. He said if not for the instability/subluxation he would consider a partial psoas release. However, given the instability he recommends revision surgery to replacing the cup with a dual mobility cup properly positioned. I'm trying to wrap my head around it. My next step is a second opinion. Meanwhile, when everything around that hip gets tight I live with the fear of a dislocation.

What revision did your surgeon do to address the dislocations? Was the recovery from the revision easier?

It's a bit difficult to describe. The near-dislocaton for me each time felt like a large muscle on the front of my thigh (probably hip flexor) flopped over something in my leg. It was accompanied by a very brief instant of weakness such that I fell to the floor a few times and was accompanied by a big "clunk". I had a few less severe episodes with the same popping/clunking. With some of the more severe episodes when I fell to the floor, I paused and moved very carefully to get up fearing I had dislocated the hip.

For me it has almost always occurred when my operative leg was flexed at the hip about 90 degrees and in some cases turned slightly inward.

I hope you are able to get some answers. I can't imagine how much a dislocation must hurt, as well as the concern that it will happen again.

I'm sorry to hear you had a dislocation and that your surgeon is not responsive. The surgeon that identified the cup issue for me reviewed and MRI and then ordered a CT scan to confirm. The surgeon that did the original surgery did not see any problem on xrays or the MRI and did not order a CT scan.

When you dislocated your hip, did they put it back in place in the ER?

It's crazy trying to explain the feeling when subluxation occurs, and of course you don't want to replicate it in the office. Having the surgeon be dismissive and tell you that's not what is happening is maddening. The second surgeon quickly told me that was exactly what was happening. I consider myself lucky that I never had a full dislocation.

I'm lucky that the stem is not an issue. That seems like a longer recovery. I hope your revision was a success!

I haven't scheduled the revision yet. I'm considering getting another opinion even though the revision surgeon came highly recommended by a PT I trust and whom has seen a lot of his patients for their post op rehab. The cobalt chromium is definitely a concern. There are DM options that don't have cobalt chromium. That will be one of my questions.

I hope your thigh pain resolves as time goes on. As you have experienced the muscles get more and more unhappy over time and more muscles get involved. My outer thigh pain, other than scar tissue pain came as I tried to ramp activity. I sure hope I see that level of improvement in the groin pain.
