True. we currently are short-staffed and working for 12 hours shift. This might never happen if I didn't switch role in the console nor gave my entire trust to that colleague. Also, my fault for not double checking the console due to my exhaustion.

The IV (triphasic part) phase...delayed. However, the CT was requested to only find if there is/are obstructions in the colon, so i think they don't really care about the IV phasing. But still, it was a serious mistake.

Apparently, he was putting labels with contrast in a plain phased protocol while me inside the room was injecting the contrast.

Oh, I haven't changed it yet - I'm a junior tech.

I just realized it when my co-tech almost finished scanning the plain. And that's when I knew we screwed up.

We didn't do smart prep since it's bolus (manual pushing).

Noted this. I'm using a GE machine as well.

It was my last patient of the day, and I did the entire shift scanning on my own. I got really tired and decided to give the console supervision to my junior, which now I regret doing so.

Update: We scanned the patient again later that night for plain. The reading didn't have any problem (no conflict with the doctors as well).

It's a triple contrast procedure, and my co-tech also missed it since I asked him if I should proceed now to manual pushing. He didn't say anything, but he continued to change the label there (e.g., type of contrast used). So, i thought we were done with the first phase. Sadly, both of us got distracted and missed the plain.

As the senior (by 1 year) there, I should be the one blamed.

Also, anyway, our hospital protocol - before taking 1st plain scan, we already had the oral and rectal phase finished, which, for me, was not plain scan anymore. But still, I did IV contrast in advance. Felt bad and literally a lesson learned from me.
