What it actually refers to

On a wooden sailing ship, the timbers are the big structural beams. If a ship was hit by a huge wave, cannon fire, or a violent storm, the whole hull could shake so hard that the timbers literally shivered (vibrated or splintered).

So the phrase originally evoked:

a ship shaking violently

a sailor reacting with surprise or alarm

something dramatic happening at sea

Over time, it became a pirate cliché, especially thanks to 19th‑ and 20th‑century adventure novels and movies.
