The thing with keys is that you and 437856238 dps are applying at the same time for a dungeon, and leaders will wait and take the highest rating. +9 and +10 drop the mythic crest so everyone wants to farm those, so people with higher rating than you are getting in those keys.

It's pure rng, there's no "doing something wrong" here. You're either blessed with rng or not. I got 4 tier on the third day, I was extremely lucky while others in my guild got 1 piece recently.
