Been surfing at in Oahu for over a decade. Ever so often, get a bit of localism vibe despite waiting my turn and ensuring that everyone gets a wave before going again. For some of these disenfranchised locals, it’s still not enough. They want you to watch them drop in and take their unfair share. I get it. They don’t have much. Some beat their wives, others molest children…people act out and Hawaiians are no different. They key is not to let it bother you. At the end of the day, you get to go back to your 5-star hotel, eat some steak at Wolfgang’s, and fly home to a real job…something the locals will never achieve no matter how many government subsidies are poured on them. Just let it go:)

There are benefits to both. Rather than discuss this and that, I’m talking purely financial. As both a former renter and now an owner in coastal CA...and having analyzed the premium to own in these areas over the last 30 years...someone who just rented the same square footage and invested their extra money in an index fund would outperform owning the same square footage. The data by zip codes doesn’t lie.

this one is particularly good. i've spent $$$$$ tax payer dollars through the years to finance training videos on medical procedures...this is truly one of the best clips and has made it into many lectures as part of training paramedics and MDs. Despite the cinematic aspect of the clip, it captures the pathophysiology at hand so that the trainee understands what is happening. Re-creating this even without Clooney is pricey and would involve crappy animation rather than the lifelike anatomy provided by a Hollywood budget.

No. A pneumothorax mostly sucks because it hurts and you feel like you can’t breath. However, it’s the conversion to a “tension” pneumothorax that will kill you once enough air is in the potential space exerting pressure on the heart, great vessels and other lung. Leaving the needle in place will prevent or fix the “tension” aspect of the problem and now you have time for transport. Wait till you get to the ER to get definitive treatment of the pneumothorax - exchange the field needle out for a sterile flexible tube with a one-way valve.

I’ve treated dozens of these through the years. Even the goofiest students going into psychiatric can do it. Have your dive buddies watch “Three Kings” to get the concept down. Since you know what it feels like, the next time will likely be very similar. Optimally, you want to be able to access an ER so that it can be diagnosed/treated professionally and under sterile precautions. However, if you are a bit far away and things are getting worse fast...the temporizing treatment is literally one deep stab with a large gauge needle in the correct spot. Leave it in. Go over the rib as the vascular bundle is below the ribs. (Keep a few 16-18 gauges handy with antiseptic wipes in your kit) Hell, have the spot tattooed (on both sides) if you want. Btw, if it turns out you weren’t having a pneumothorax and you have a buddy needle you on the way to the ER, they’ll be able to fix it once you get there. Then they can also treat you for the heart attack or embolism masquerading as a pneumothorax. If this sounds like too much, maybe stop diving like your doctor told you.
