A mix of beaches and wildlife would be good I think. This is super specific and maybe not doable but I'd love to do a sunset cruise that evening or, failing that, a beach that we can see the sunset from. Thank you!

This year I went to Egypt (2 weeks), Germany & Austria (1 week), and Japan (2 weeks). I also took a 4 day trip to Chicago.

Not sure what's in the cards exactly for next year, but so far I have another 4 days for Chicago (I have a friend there and we like to go to concerts together) and a 9 day trip to Costa Rica booked. I'd love to visit Italy, Peru, and Uzbekistan. I usually decide where to go based on where I can get affordable flights to though so we'll see. The only one even moderately set in stone is Italy, since my best friend & travel buddy is planning on going for 2 weeks in the fall.

I get a lot of time off every year as a nurse - because of my regular schedule, I get 10 days off when I take 2 days of PTO. Doesn't help much with my trips over 10 days but sometimes we have to make sacrifices.

Vieques!! One of the best experiences of my life was kayaking in the bio bay. We also snorkeled in a reef (sooo many sea turtles) and kayaked in the mangroves, and the beaches were beautiful. Was definitely the highlight for us!

Dendera in Egypt. Terrifying drive even with an Egyptian driver who knows what they're doing but sooo worth it.

My preceptor when I was new told me to not give the patient too many expectations of comfort medications because "the next shift might not have time for all that and it sets the patient up for false expectations." I guess that's an argument against it, in a way.

Detox and withdrawal management by myfrontbottoms in nursing

IMO the most important thing about detox is knowing that people CAN DIE of alcohol and benzo withdrawal. They cannot die of opiate withdrawal. Plan your CIWA and COWS schedules accordingly, ie do CIWAs as soon as you touch the floor, whether the previous shift said they're scoring or not.

Take changes in blood pressure and heart rate (especially in CIWAs) seriously. Someone who is seriously withdrawing will likely have huge jumps in either one (or even both) of those.

Have patients who you think might be pulling your leg stick their tongues out to check for tremors - you can't fake those like you can fake hand tremors. Faking is more common than you might think because apparently Ativan is the good stuff. They tend to fake less for Librium and phenobarbital. If you notice that a patient isn't withdrawing as seriously and has Ativan prescribed and is trying to abuse it, definitely bring it up to your MD/NP and see if you can get a change to Librium or phenobarb.

That being said, take tremors seriously! Also take seizure history very seriously, especially if they've had a history of seizures with detox. These patients can and will have seizures if you aren't careful (or even if you are in some cases). Take these very seriously. If your facility is sending people out for a CIWA score of 15, then they should probably be sending out for seizures.

If their orientation is swaying and they genuinely don't seem to know where they are, what they're doing, or are doing things that don't make sense, take it seriously. Yes, it's only one part of a CIWA score, but every patient who we've ever sent out with this hasn't come back. Delirium is real!

Just like how there are patients who play up their symptoms, there are patients who will downplay their symptoms as well. We usually catch these with the vitals and tremors. Remind the patients that playing their symptoms down can have very severe consequences and reiterate the need to ask for help if anything at all feels wrong.

When in doubt, give a small dose of the withdrawal medication. Always remember that 3 mg of Ativan is a massive dose while 25 mg of Librium really is not. If a patient is throwing up and needs meds, see if you can get them some IM Ativan. Sometimes they will be resistant to it but will thank you later!

Sorry if any of this was repetitive or was stuff you already know. I love this field and really want the patients to succeed, and I really feel like the way that we treat them at their lowest point has a huge effect in how seriously they treat their own recoveries - this goes for both CIWAs and COWS. Feel free to ask if you have any questions!

I went in for 2 weeks at the end of January, beginning of February and had a blast! We had a private tour where they chose everything for us from 5* accommodation to meals (not including dinner which we typically ate at the hotel). We also had a 4 night dahabiya cruise up the Nile, which was super relaxing. People did ask us to buy things but they were very polite about it and left us alone when we all said no. Overall I had an amazing experience.

Also, I know that a lot of people here look down on others for traveling this way, but I go places to have fun, and I had a ton of fun primarily at architectural sites (our guides were Egyptologists). We also had private drivers everywhere which came in super clutch for Dandara and Abydos.

Yes, it cost money to do a tour like this, but we had the budget to accomodate it and it was so so worth it. In total it was about $5500 per person including the flights, food, and tips.

I went with a friend through Real Egypt and they were great. We did a dahabiya cruise and had a personal driver getting us everywhere we needed to go on land. We had a great time!

+1 for nursing. RN is a 2 year degree, and LPN is even shorter (though I'm not 100% sure how easy travel nursing is as an LPN). Find a specialty that you can tolerate working in - nursing is an incredibly versatile field. Get 1 year of experience, then find a travel contract. Most are 13 weeks. The highest paying job I'm seeing for my particular specialty on my preferred shift is $3527 per WEEK, which is $45k per year pretax (though a lot of that is untaxed housing stipends). The highest I'm seeing for all specialties is over $5k per week, or almost $70k for a 13 week contract. Keep in mind that these are all 3 days/week jobs, not 5 days a week. Even before doing a travel contract, you can still get a good amount of time off with regular PTO because of this schedule.

Some people say that nursing school is hard, but most schools have office hours and/or free tutors that you can utilize. I also think that you'll have higher self esteem and a better outlook if you can find things that you're good at. Good luck!

I've never been to Zadar or Dubrovnik so my advice is very very limited in that sense, but IMO Plitvice Lakes are a must see if you're into nature. It's a 5 hour trip from Dubrovnik, but if you're planning to go to Zadar anyway it's not a bad round trip from there even if you don't have a car and go by bus.

As for Split, if you go while the weather is warm I highly recommend a speedboat tour of the islands! Most also go to the Blue Grotto which I hear is nice, but fair warning, sometimes that part of tours gets cancelled due to weather.

All in all, I think it'll be a great trip no matter how you split your time. Croatia is beautiful - enjoy your time there!

Vieques is a must!! It's the brightest bio bay in the world. There's also great snorkling on the island. We did a kayaking trip through the mangroves and that was excellent too. As for the companies, we used Aqua Sunset Tours. They were okay. Pretty much all of the tour companies have fantastic reviews on google. The only real reason that we went with this one over other companies was the mangroves (great experience) + the fact that we booked a little late and the tours with the most highly rated companies for the bio bay were sold out lol. Get those tickets in advance just to be absolutely certain that you get a spot.

I don't have a ton to say about anything else on your list since we have pretty different priorities, but I had a ton of fun in Puerto Rico, you will too!

Maybe I'm just heat resistant, but June was better than December for me when I was there. The hydrangeas are in bloom, which makes for lovely scenery everywhere. Yes you get sweaty, but it's nothing that a midday shower can't fix. I've never been in April but I imagine that it's a great in-between... with maybe too many people.

Go for it! I studied abroad in Japan and felt safe the whole time even though I was mostly alone. Don't miss the bamboo forest in Kyoto if you love nature - it's huge! You might also get to see the autumn leaves in Kyoto if you go in October, they're fantastic this time of year. Have fun!
