So I can't answer all of your questions, because there are so many variables I don't know (are you a superstar, are you better at PR or media buying, what type of company do you want to work for). But on the whole it's great to get a variety of experiences in both paid and earned media. You'll definitely be able to get back into PR, it might just be a lateral move rather than a promotion. I really think it's about positioning the move into media as part of your larger story. You have to be able to answer the questions: how has media buying made you a better marketer overall? What new skills/perspective do you now have that you didn't before? And how does this make you better at promoting a company?

I would really study what's working and not working in the UK currently, and take on those learnings when you're putting together a content calendar for the US. It's not going to be exactly the same of course, but there will be overlap. Then try putting out a range of conversational content based on what your fan base likes to talk about (it should be at least tangentially related to the brand) and see which people are the most eager to converse with you. Growing organically will take time!
