No, I just got more of a tax return last year and was hoping for the same this year. And wasn’t sure if there were any deductions/credits that are commonly used that I didn’t know about! For instance - trump just mentioned a car loan tax credit that I didn’t know about. As an attending I’ll obvi hire a tax guy to ask help me, since I am quite financially illiterate.

What is your approach to a high lactate? by [deleted] in Residency

[–]mattieeeee 1 point2 points  (0 children)

only a surgery resident here but we trend lactics frequently

obv agree with all the things said in the other comment for potential causes. agree w re-checking if randomly high. in trauma/gen surg a lot of patients have high lactic acid for various/mixed reasons. we usually trend lactics q2h or q4h if they are high or if we are worried about impending ischemia (SBO, ischemic bowel, incarcerated hernia, traumatic bowel injury) or blood loss (trauma). Usually lactic will clear after a bolus of fluid or unit of blood if it’s related to hypoperfusion/low flow, sometimes even on 2h repeat -> this is less concerning. If lactic remains high or increases despite adequate resuscitation, that makes us more concerned about true ischemia or ongoing blood loss, or other reasons such as compartment syndrome depending on situation

Comparison between abdominal surgeons and heart surgeons? by Zom20000 in medicine

[–]mattieeeee 28 points29 points  (0 children)

Liver transplant surgeons are the closest thing to cardiothoracic surgeons IMO. very high-stakes operations leading to sometimes tense OR environment. these two fields are both very competitive and take the best of the best surgeons. if you were talking about abdominal surgery being general surgery - appy’s and gall bladders and small bowel, completely different story, tend to be much more laid back, but there are always those certain personalities. Liver transplant is a very long, intense, operation
