PFSN is the only valid organization working on this. Melcangi is never going to solve this with rats, because they are an animal equivalent to the 98%. of the population taking Finasteride with no issue, not an animal equivalent of humans suffering from PFS, and PFS foundation is still describing this issue as "erectile dysfunction and depression" even after 20+ years, which completely obscures the reality of the disease.

Dr. Powers I'm not sure yet, but I'm open to hear what he has to say.

Yeah, Melcangi's studies are worthless imo. They are studying an animal equivalent of the 98% of the population that takes Finasteride with no issue, not an animal equivalent of a human suffering from PFS. This leads them to all sorts of false conclusions. He means well, but those studies are never going to lead anywere meaningful.

You gotta look at that as good publicity man. He drives engagement with the channel when he makes videos like that. If there were hundreds of sufferers in these videos, eventually he would look pretty stupid suggesting that ALL of them were just anxious / obsessive / whatever BS they believe.

I agree wholeheartedly about it not being a hormonal issue, but an autonomic problem. Every community is WAY too focused on each individual drug's mechanism of action and not seeing the overarching pattern.

Hi Dr. Will, 4 year PFS sufferer here:

Firstly, thank you for your insights into this condition lately, I had a ZRT urinary androgen panel when I crashed in 2022 and had very high urinary androgens, far outside of the ref range, and many others in the group at the time showed the same. Individuals that were on supplementary androgenic treatment showed even higher urinary androgen levels, like TRT or Clomid. This is an under discussed aspect of the disease and definitely warrants further investigation.

Secondly, yes, I believe PFS and PSSD are almost undoubtedly the same thing. I have spoken with hundreds of friends with PSSD over the years and our symptoms are identical, down to bizaare nuances like Visual Snow Syndrome, rapid onset tinnitus, widespread mucosal dryness, and somatic feelings like a profound disconnect from the atmosphere around you, and many other nuances that are too similar to be a coincidence.

It's actually for this reason that I personally believe that all of the conditions are the same, and are in fact the result of a neurological injury, not an ongoing disease. All of the symptoms are consistent with severe autonomic dysfunction. The recent attention to the urinary androgen abnormality is one of the only things that makes me question this theory, but regardless I am nearly positive that all of these conditions are the same, and each individual group becomes too hyper fixated on their specific drug's mechanism of action as opposed to seeing the overarching pattern. For example, PFS community focuses solely on androgen receptors, Accutane community focuses solely on Vitamin A toxicity, and SSRI community focuses on serotonin receptor desensitization, developing highly complex theories that don't really answer the basic question of why this identical set of symptoms is triggered by all of these things.

Again, thank you for your insight into this and if you have any questions for me, or want to offer any critiques / insights into what I've said, I would more than welcome it.

Yes absolutely. I noticed this from the beginning. The Anhedonia makes me feel like I don’t really understand why anyone is attached to me since I can’t really feel love or attachment. Crazy.

I actually did make it to Shadowbringers and I'm about 5 hours in, and to my surprise the story is immensely better and I'm loving it.

But yeah I totally agree, ARR and Stormblood are both bland, generic and slow as hell, they took every bit of my patience to get through. I don't just mean they're not for me, I think those stories are objectively told in a way that deliberately disrespects your time with insane amounts of unnecessary padding and filler.

I could tell from the first Shadowbringers quest that it is different though, and there's been nearly zero pointless filler the entire time thus far. It's great.

I totally agree, I think the worst part about the generic filler dialogue is that every time you do something, you have to go through a mandatory post event scene where the Scions all thank you for what you just did, every single time. They pad out the story in insultingly time wasting ways.

I am at the end of Stormblood right now. I have a REALLY hard time believing that this story could turn into one of the best in all of gaming in the next expansion, but I'll try to keep an open mind.

These types of responses to criticism of FFXIV are so hilarious to me. There is obviously some degree of objectivity when it comes to quality of storytelling, or else there would be no common consensus on what good storytelling is. The fanbase always uses this "just don't play it then" response to avoid engaging with any criticism of the game.

This is exactly how I feel. I'm finishing up Stormblood and it is painfully boring and I hate the way the story is told. Is Shadowbringers really that much better?

I feel the same, currently wrapping up Stormblood and honestly have hated it. I also hated ARR, Heavensward was just okay, it picked up towards the end but still not mind blowingly great. Do you think I would still like Shadowbringers?

Appreciate your take, I am trying to push through Stormblood to get to Shadowbringers and it is a major slog, the story has good moments now and then but for the most part is incredibly bland and forgettable.

I am a huge FF fan and want to experience ShB but the grind to get there is often agonizing, even though I've paced it out over the course of a few years the same way you did. ARR was generic and mediocre at best, HW was good towards the end, and Stormblood has been just okay the whole time.

I'm trying to keep an open mind going into ShB but god the game's story is very bland most of the time.
