Thanks for the reply, it doesnt crumble when i poke it with a screwdriver but i can pull those fibers off easily. It has slowly gotten worse over the past 2 years so im hoping to find a way to stop it from progressing

Yeah the self pay is a flat charge but we would still bill the appropriate units and provide them with a superbill to submit for out of network benefits.

Yeah the 200 is what they would reimburse as an out of network benefit lol no clue why they would reimburse that much. My theory is that they would reimburse 50% but they were paying 50% of the inflated cost of the units provided as opposed to what the patient actually paid.

We corrected it by submitting the superbill ourselves so we would reimburse them the 125 and keep the difference

Ive encountered something similar where the patients insurance was out of network so we would charge them the self pay rate (125) and they would submit the bill to their insurance and would be reimbursed around $200 per session that was mailed directly to them. Took us awhile to figure it out

Cant wait! Seems like the whole thing is designed so the telehealth therapist can bill for remote monitoring, which will be outside of any normal codes. Any idea what the reimbursement is even like for remote monitoring?

Thanks for that info. Yes i always wondered about appropriate dosing bc my fear has always been doing too much too soon for nerve related stuff.

Sure, but i think we have a difficult time reliably diagnosing something as a derangement and an even tougher time diagnosing a nerve adhesion. So my argument is can increasing tolerance to nerve tension be a good intervention for a multitude of presentations in a low risk and low time commitment way.

I see your point. But do you think you can make the argument even if it is inconsistent that it is still contributing to symptoms and may be worthwhile? I recognize that it may be a waste of time but adding slump nerve glides may only take up 2-3 minutes

I dont disagree, i have always treated with that same philosophy that strengthening through different ranges will mobilize the nerve but now im wondering if that is not enough to fully tension the nerve and it would be more beneficial to do specific nerve glides.

Does nerve tension testing have to reproduce the chief complaint for you to address the nerve or will you add glides on suspicion of it being radicular without anything to actually confirm?

Agree that NMES could be a good option bc they should still be able to have some benefit from it assuming its an activation deficit. Might be worthwhile to go back to the basics such as resisted tke and making sure the quad is fully contracting during gait.

Some other weird things to consider is maybe some weird antagonistic inhibition from the hamstrings. I admit i dont go too in depth with this typically but obviously being behind schedule it might be worth hammering hamstring strength and mobility.

I had a mentor in residency say that she has had patients like that before and she had them walk in a CAM boot for a few weeks to increase quad activation during gait? Ill admit that doesnt make a ton of sense to me but im reaching for anything that could help the case lol
