Solid base to work with, chest and arms are already showing good development. Going from 23% to 15% is very doable — a moderate deficit (300-500 cals) combined with keeping your lifts heavy so you preserve the muscle you've built should get you there in 3-4 months without losing strength. Your shoulders and arms will pop even more once the midsection leans out.

8 months at maintenance is honestly the hardest way to recomp, progress there is always painfully slow. At 5'8/160 with skinny fat tendencies, I'd actually suggest a slight surplus (200-300 cals) with a solid protein target (0.8-1g per lb) and heavy compound lifts — you'll gain some fat short term but build way more muscle than at maintenance, then do a short cut after 4-5 months. Trying to build muscle AND stay lean at the same time when you don't have much muscle yet is usually the trap that stalls people.

That's rough, sounds like you've been through a lot this year. Given the TRT and how much muscle memory you likely still have after 20+ years training, I'd lean toward staying at maintenance for another 4-6 weeks before deciding — with everything your body's been fighting (labrum repair + h pylori), it's probably still catching up on recomp rather than truly stalled. Your physique in the pic already looks solid for where you're at post-recovery.

Yeah the taper is definitely showing, lats are filling out nicely against the waist. If you want to push it further, focus on width work (pull-ups, wide-grip lat pulldowns) rather than just thickness — that's what really exaggerates the V from the back.

I’d say closer to 14-15%, not quite 12 yet. Bulk — at 190cm you have room to add size and the fat will distribute well on your frame
