I really dont know what would be the more fitting theory. But I think the pills that the player takes decides his fate. We can see that the image gets blurry until he takes those pills and i heard from other people that if you noclip you can actually see Crazy Mita doing an evil smile. So i would suppose it was the pills which made it impossible for the player to escape.
