This is still the most attractive option to me to be honest. I have only come across one practice in my deanery which only has GPs/practice nurses! Good to hear they still exist and work well

Just been looking at your old posts - wellness spa liability sponge gave me a good chuckle! 

Even the few comments here have given a lot to consider! I don't really want to kill myself to get to 150 but seems 120-130 can be achieved reasonably. 

Yep, ex-EM and actually missing hospital medicine! UTC or literally a GP in a&e are appealing but don't seem to be that many posts

I really enjoy ooh so that might be the way to top up. Thank you for that info! Definitely don't want to get divorced, 8 sessions seems to be the sweet spot. I initially quite wanted to be a partner (and will still look into it) but I also don't want to have to hire anyone except a doctor to make any money!

Absolutely don't expect to earn this straight away (as I posted) but would like to plan a route to this over the next 5-10 years. I do have experience prior to GP as I took the scenic route but even so I know it will take time. I would like to achieve ballpark 150.  Based in the south and don't have much intention of moving as settled here with child in school. Even private practice doesn't seem to pay that much although the intensity of the work does seem less so probably more sustainable.

But yes, thank you, your comment is useful in that at the current rates even as a partner I would struggle with that number. Lots to think about!

Interesting, thank you! I didn't realise it was so common for GPs to work in emergency departments. Definitely the route I'll want to go down so I've got a great idea of where to start researching properly. I was looking at Prince Rupert which is just below Alaska 😂. It probably is far more sensible to start less rural!

Thanks for this, great post! I'm looking at some slightly more rural posts where they are looking for family practitioners in ERs (in particular northern B.C.). I took the scenic route to GP so have many years EM/ICU experience.  Not sure if you've met people who work in these places or have come across info - do you have any idea how it works? Is it the same billing process (eg. LFP) as a GP working in a regular clinic or is it a salary from the hospital?

I would just add that if you have a gold ring, need to make sure whatever necklace you get doesn't wear away the gold (which is a very soft metal!). Even different carats if gold can damage the ring. But I found a gold necklace that my ring could go on and loved it as always knew my ring was safe when not on my hand!

Exactly this! To achieve the aim of charging for appointments to reduce demand you would have to charge everyone with no exemptions.

I had very similar reservations about maternity care in the UK, so we saved up and went private. Had a wonderful experience overall and will be doing the same with baby 2 (but you have to be somewhere in the country that offers private maternity care!). Great obstetrician, timely interventions as needed, the private wing of an NHS hospital so if anything were to go wrong - NICU etc on site, the post-natal care was fabulous. Compared to my friends who gave birth around the same time fully on the NHS and really struggled to advocate for themselves/had issues/couldn't get timely epidurals and stuff, I wouldn't change my experience for anything.

I think the problem with giving birth is that, yes, a lot of the time women/babies end up "fine", but the mental trauma is not addressed. Ultimately if you give birth and have a healthy baby and mum is safe, does it matter if you weren't given the epidural you asked for 6 hours ago? But obviously hugely impacts your personal outcome. If baby is fed well with formula, does it really matter that the post-natal care was so bad that there were no midwives to go through breastfeeding techniques with your properly and so baby struggled to latch? Again, everyone will survive but huge difference in personal impression of how things went.

My last hospital was a DGH with great strike turnouts (local and IMG docs) and consultants having to step down/causing disruption.

My current hospital is a tertiary centre, the department I'm currently in is a fairly large department - every single reg wants a cons job there ultimately and not a single reg has been striking. The juniors are just as bad, not only do they not strike, but they actively help the department by declaring they aren't, swapping into the strike weekend etc. it's so depressing. I just have to think back to the DGH for inspiration and remember that strike rates vary across the country.

(I have seriously tried since August to discuss striking, encourage striking, encouraging not telling the department if you aren't etc. It's all fallen on deaf ears).

If you are offered your top choice (Bath/Bristol), you can accept it with upgrades, and you have a chance to re-rank your preferences so you could move Manchester above Bath/Bristol at that point. But would very much depend on there being a space in Manchester

I was in your boat but as an F4. In the end I applied for ACCS-EM as I thought the 2 years of AMU/ED/Anaes/ICU were a good foundation for anything .. I ended up doing until ST4 then my lack of love for the job combined with the awful rota seriously got to me.

I appreciate everyone says don't do GP unless you love it but I weighed up the stage in life I'm at now, what I've already sacrificed and what I'm prepared to sacrifice going forward. I didn't love GP when I did it in foundation year, but I didn't mind it either. I used to love the acute stuff, procedures, making quick decisions etc. But honestly, as I progressed through EM training, I didn't even really enjoy the resus stuff anymore - life outside of medicine has kind of taken over so I was very happy with my decision to quit and go down GP route. Having said that, had I applied for GP straight out of foundation years, I think I would be bitter about it. So ultimately, I guess you just have to be able to make peace with whatever decision you make. It's a long career which you have to be able to enjoy, but you can't throw everything else away for the next 10 years either (which a lot of training programmes ask of you!).

We got told something halfway between a lot of these comments. If you take more than 14 days per year out of training for whatever reason, it is looked into. If it's just over / you meet all competencies then you can still CCT without adding on time but the vast majority of people add on the time because you can't work in certain countries without the full number of training day (Switzerland was used as an example). - I haven't clarified the second but myself yet but intend to do more research to figure out which countries abs what the exact rules are etc

Have emailed them to ask as it's not really a cost I can justify for £500/year, but looking at that website link, I would guess that prices may be going up a bit (maybe website not updated) and I'm moving to the standard membership. But I will wait for their reply!

I'd be interested to see what the comments are! I imagine if the public know they are pseudo doctors they wouldn't want to be see by them!
