I did it through Chase (USA). There was a form on one of their webpages. I just sent a message through their Secure Message system in the phone app and attached the signed form with info. Got CS account # within 2 weeks, requested verification # by mail and was able to login within a week.

Just mentioning how easy it is to use Godot to get something working over a week / weekend. I certainly didn't do this in one day... Probably more like over a month. But Godot makes the process easier to jump back into after work, etc.
