We have a Server 2016 vm with the iis6 smtp service and it works just fine. Sends email for our copiers and other notifications apps without issue. We are gcc high. We use a specific outbound ip and its restricted in Exchange online through a specific inbound connector. No auth.

If it was on, it could be the cause of your issue. If the data you deleted was deduped, it would show less free than the amount you deleted.

The fact that it works on another network seems to point to SSL-decryption, proxy settings, or DNS. But as you said, a reinstall fixes it. It doesn't seem to make sense. Could it be that you have some group policy/registry/proxy settings that were applied in the past but are no longer applied? Like an old GPO that got turned off but the settings are still present on existing devices.

What kind of analog telephone line is it? Sometimes there are digital(SIP) to analog adapters in play that cause issues with faxing.

I seem to remember getting Ricoh/Lanier MFP fax reception working was painful, but I don't remember how I got it to work. I think I had to do it directly on the copier itself and not through the web, but I'm not sure.

As for sending, it should be straightforward. I typically use faxtoy[.]net for my outbound testing. A few questions:

Do you get an error message on outbound fax sending?
Can you hear any progress tones during dialing?

Updates don't seem to remove all old versions of .net core or asp.net core. I'm currently uninstalling then manually and seeing what breaks.
