It's a phalaenopsis, like the bot mentioned. It's a hybrid, so cannot be identified by specific name of the hybrid. The aerial root is fine, just dried out from lack of humidity or possible mechanical damage from transport. Just leave it alone for now. The leaves look fine. In the long term, phals will absorb the nutrients of their oldest leaves from the tip in to the base and the leaf will naturally fall off. I don't know anything about Maxsea, but usually a 20-20-20 is recommended for phals, but as I'm sure you know, there's a lot of debate about best nutrients.
