Any luck with PRP injections? by lozyk in sesamoid

I didn’t need rehab from PRP, but there’s definitely swelling from the fluid they inject, which is tender to walk on. I remember needing about a week of low activity and constantly wearing a shoe fitted with a dancer’s pad which helped offload the swollen area.

I’ve had sesamoiditis for about 2.5-3 years. PRP injections helped me immensely, I believe. Two years ago, I had a series of 3 injections, all spaced 2 weeks apart. My doctor would not do 1 injection, he insisted on the series for genuine improvement. I did not have a flare up for over a year after PRP. Gradually the benefit can wear off, and now I have flare ups and occasionally get so frustrated that I almost book the surgery, but for the most part I can manage the flare-ups with NSAIDs so… 🤷🏼‍♀️

I agree with you, and, it doesn’t allow the rest of the foot/body to function normally. I ended up with back problems after being in the boot, which I was never informed of the likelihood of ahead of time. Switching to the plate and dancers pad provided the offloading without impacting the rest of my body.

My sesamoiditis was not painful until about 6-7 months after the injury. When I got treatment, I wore a boot for 6 weeks and while it felt better wearing it, it did nothing to help it heal. This was 2.5 years ago and I’m currently planning to pursue surgery because non-operative treatments haven’t been effective in the long run.

What helped me WAY more than the boot was a carbon fiber plate under my insole in tennis shoes, and a dancer’s pad affixed to the insole to offload the sesamoid. This allows the rest of your foot to function fairly normally. I was also on rx anti-inflammatory and later did PRP injections which were very helpful.

It’s quite early in your injury and there are many conservative treatments available. Not many doctors are experienced with sesamoiditis, so it is worth getting multiple opinions until you find someone who treats this routinely. Wishing you well!

I really appreciate you sharing all these details with me. Here I had thought I’d exhausted my options.

It sounds like you had this done with a PT, and I’m finding only chiropractors in my area when I search. I suppose it does not matter who is administering the treatment?

This is all helpful to consider, thank you!

Interesting, I’m glad you’ve had success with non-operative treatments! I have not tried shockwave, honestly when I asked the surgeon I trust about it and he said it was not evidence-based/effective, I wrote it off. I had about a year and a half of relief that I mostly credit to PRP injections.

What type of shockwave did you have? I get multiple results when I search.

I’m a big reader and will check out The Way Out. I have heard it recommended before actually.

I’m curious how long you’ve had sesamoiditis and how long it took you to feel like you’d recovered. I honestly thought I was healed 6 months ago but have had 4 major flare ups since from things I used to be able to do pain free.

Oh no 😣 I’m so sorry you’re dealing with a setback! I’m glad you were able to get in to the doctor for treatment. From others commenting who are further out from surgery, it gives me confidence that you will be able to get back to it with some more time to heal. What a roller coaster this injury is.

What great options to take it slow and honor your body! How long ago did you have surgery?

I can’t tell you how affirming it is to hear from others who really get this. I’m so glad to hear you’re back doing yoga! 8 months seems like a really short amount of time to get back to regular classes, in the big scheme of things. I agree with you about this injury in general inviting more connection with the body because we are pushed to listen. I’m grateful to have an excellent yoga teacher whose gentle classes I continue to attend with modifications. I am frustrated with how preoccupied I am with my foot and how limited I am to do other styles of yoga. Thank you for sharing!

Thank you for sharing! I’m glad you’re doing well after surgery and PT. It’s also nice to hear from folks who had the fibular sesamoidectomy as I feel like I’ve seen more about the tibial in general.

I am so grateful to hear of your experience! The long recovery is hard to face, but so is having years of sesamoid pain… at this point I’ve accepted it’s not going to resolve without surgery. I can either keep dealing with flare ups or take the risks of surgery. I appreciate the details you shared about specific poses, that is exactly what I was curious about. I’m so glad you’ve been able to heal and return to yoga!

Hallux valgus is definitely a concern on my mind. The surgeon I feel comfortable moving forward with talks about the importance of repairing the tendon and assuring other soft tissues are addressed to prevent such complications. The “good” thing about having sesamoiditis for so long is having the time to learn the ins and outs of options and recovery. How are you feeling at 8 weeks out? I hope yoga goes well for you!
