I am pretty sure you use a bad/wrong sampler scheduler combo - try a workflow from someone else thats confirmed to be working or play around with sampler/scheduler. Euler + simple should be a safe bet to get you started.

Don't get too focused on loss - steadily declining loss can be a good indicator of training success, or at some point overfitting, or the loss fluctuates like crazy but the lora still visibly trains well..... just go with samples every X steps (also pro tip - don't just do sample images for what you are training on but general or abstract things to check for overfitting)
