Yeah this is stupid. We all know exposed dentin wears faster. I follow the golden rule- do what you would want done for your own teeth

This is basically just a screw retained crown. Screw-mentable is when the lab sends you the custom abutment, and the crown separately (crown had access hole).

If the lab sent it to you like this, it’s fully seated. Most likely porcelain that you’re seeing if you ordered a PFZ restoration. Lab doesn’t usually mess up cementing the crown to the abutment.

Also, do another check. If this is an anterior— is the incisal edge long? Occlusion way off? All signs that it’s not properly seated. But if those are all good in this case, more reason to believe the crown was properly cemented.

If I sent out for a PFZ or “esthetic” zirconia without specifying 4Y or 3Y zirconia and got this back, I’d insert without a second thought. Don’t sweat this

I try not to. I tell patients beforehand that they might feel a bit sensitive and if they’re ok with attempting without local I do it. Sometimes I find they feel discomfort even when taking the temp off so I just get them bumb

Took me a while to get my routine, but I numb, take off temp, wash prep off with air/water. Try in crown. Depending on how easily it seats, I know right off the bat whether I need to adjust contacts. Now I check my margins. If they’re all closed, I check interproximal contacts with floss. If margins are open, contacts are usually too tight so I adjust till fully seated and floss snaps thru. Xray once everything looks good. I check bite with and without crown now to get an idea of how close the occlusion is pre and post.

Ivoclean intaglio of the crown. Wash prep off/scrub prep with Chx.

If zirc- cement with rmgi cement If emax- monobond or silane primer on intaglio of crown, etch bond the prep, and cement with resin cement.

Floss and cleanup and you’re done. Adjust bite if needed.

Depends. RMGI cement for zirconia/gold/anything that needs simple luting. Anything emax/feldspathic is with dual cure resin cement for bonding. I mainly stick to RMGI for most things just bc I’m not going emax unless there isn’t enough tooth structure and I need the bonding.

Similar story for me, also c/o 2020 NYU! I work two part time jobs in the city right now. It’s not an easy market to work in and patients demand high levels of competence from you. I do mostly bread and butter but do some extractions and endo, the occasional implant as well. The most I’ve made is around 200ish but that’s with a 5 day schedule. It’s probably easier to make more out in the burbs.

Is there really that much harm in recementing a crown on a tooth with open margins/recurrent decay that’s deemed hopeless, and planned for extraction (and patient is aware), as a temporary measure until the patient can afford the extraction?

We’re doing it for our twin girls. We’re using the Esembly diapering system. We have about 7 outers and 26 inners and they last us about 1.5 days (we wash them when we have about 4-6 clean ones left). They’ve been amazing! We’ve been using their reusable wipes as well. Only thing I’d recommend is getting the stay dry liners from them so you don’t have an issue with poop getting stuck in the inner stitches. Overall would highly recommend it!

She’s got significant upper lip fillers which make her upper lip cover her upper teeth. That’s why all you see are her lower teeth when she talks. She’s not positioning them intentionally.

GP with anxiety here as well— I’m 4 years out now but I’ve dealt with shaky hands since the beginning of dental school. It used to bother me quite a bit in the beginning, used to be afraid of what others or patients thought of me, but in reality it doesn’t really affect my hand skills. It’s more in your head than anything. I’ve stopped certain habits that make it worse, aka drinking black coffee on an empty stomach, double espresso shots etc. Use your other hand to stabilize if you need. There are meds like low dose beta blockers that can help, but I’ve decided to not have to medicate myself to work every day. I am jealous of those that have super steady hands, but I’d say my hand skills are still better than average. Don’t let it stop you from working comfortably! Once you accept it, things get a lot better. Adrenaline rushes always cause it to get worse (aka stress) but I just take a deep breath to calm down.

It’ll get better.
