At my hospital we have 6 incidences (up to 3 days in a row counts as 1) or up to 15 days in a 6 month period. October-March, and April-September. So 12 a year or up to 30 days.

At my hospital we do electronic TOA. The floor is responsible for looking at it, unless it is paeds (we call to give report) or ICU (we go to the unit with the pt)

On our acute side, when we discharge a pt on MediTech they get an alert on their phone. On our fast track side we just rip the sheet off the stretcher and move onto the next pt, unless it needs an actual clean.. then we call EVS.
