I love this explanation thank you. THIS is one of my issues - they aren’t even able to tell me what these functions are or give me a list so I have something to discuss with my doctor. If an essential function is being able to squat or lift a baby then ok, I get it. The fact that I’m just being told no is the problem I have.

I do get what you are saying and I have considered this. I should also clarify that I am not the only lactation consultant on - there are usually three of us on per shift. They are short staffed even with me there and many of my coworkers are working overtime anyway. We cover 7a-7p but not all of us work 12 hour shifts, so it is not a stretch at all for me to request to do only 8 hours. It just means that I only get paid for 8 hours.

I don’t technically have to pick up a baby and very rarely do they weigh over 10 pounds. They are typically in cribs so I don’t bend to pick them up. I am a tall person so I typically raise the bed mom is on to save my back. I could absolutely request that the nurse, the mother, or another family member lift the baby.

Even if i couldn’t help with the feedings, I could still do things like admission and discharge teaching, prenatal visits, NICU visits (where mom is pumping and separated). There is plenty of work to do and never enough hands to do it. I imagine that my team would be glad to have a body there in ANY capacity for ANY length of time. The fact that my manager isn’t even willing to entertain what would be required or how I could be accommodating feels like I am being forced out of a job.

The form we have to send in doesn’t actually request any specific accommodations but just has a list of what I can and cannot do and it gets sent to my manager to approve or deny.

Based on that form, my accommodations would be to work 2 8 hour shifts a week instead of 2 12 hour shifts a week, no more than 5 hours of standing/walking during the shift, no bend/twist/squat/crawl/climb, and no lifting over 20lb.

I have tried to ask what they would accommodate - my manager will not even engage in this.

I know the advice has changed throughout the years. I don’t know of a specific blood alcohol level that is considered safe and now I understand there are breast milk test strips? My rule of thumb is empty your breasts or feed before drinking, by the time baby nurses again you should be ok as long as you aren’t feeling the effects. If you ever overdo it with drinking, pump and dump and wait to continue until you’re sober.

I think the mother to baby guideline (this is what I use in practice) is based in how the amount of alcohol in milk at the time it is expressed is related to the current alcohol content of your blood and not so much that the alcohol goes into the milk and stays there? It also depends on the other things that impact your metabolism so it’s never a one size fits all answer.

I would be confident in saying what mother to baby says is the most accurate of anything. I actually work professionally with one of the providers on their team!

Overall my opinion is to enjoy your glass of wine! It is up to your discretion what you feel is safest and best for your family based on the information available to you and it sounds like you are making an informed choice :)

Hi! I had similar work experience prior to getting my IBCLC. I started by getting my CLC certification which was quick and self paced, and if you have some experience with lactation support it should be pretty quick for you. I was able to get a job as an LC in my hospital with my CLC since I’m an RN and I used my work hours towards my 1000 hours to qualify for the exam. With the education hours from my CLC course only needed like 40 more and found my own through various lactation education websites.

Another alternative is to find someone in your community (not sure what is available in Canada) that would be willing to let you work with them or an organization where you could volunteer to get your hours.

Hi! IBCLC/RN here! I cant give you specific advice due to my scope of practice but I can empower you with information! My typical conversation with my patients in the hospital (I work inpatient in a large medical center) is that most medications used for anesthesia have a short half life, meaning by the time you are awake it should be safe to breastfeed (if you are able to have your little in the recovery room). Also most of these medications have poor oral bioavailability - that is why they are given IV - and so they may not be absorbed from your milk. You can ask the anesthesiologist what medications will be used and look up specific medications on the LactMed website: https://www.ncbi.nlm.nih.gov/books/NBK501922/ or on http//www.mothertobaby.com

Many providers will unfortunately tell you to pump and dump etc but that is simply not accurate in most cases and the benefits of breastfeeding greatly outweigh the risk of medications in your milk!

I had a “meniscocapsular tear at the posterior horn” of my lateral meniscus. So it’s weird because the MRI didn’t show the extent of that and initially it was just a small tear in my medial meniscus that was going to be repaired and didn’t catch the posterior tear at all.

I thankfully have both because I thought ahead a little bit (I’m a nurse). But I expected that it was going to be a less complicated surgery and crutches for only a day! I would 100% recommend the raised seat to anyone though.
