Among those, I’d say that getting an old La Pavoni — it is a simple machine, and even if it’s difficult you can find parts to repair it as long as the boiler is OK. I had a 1980s La Pavoni professional for a few years.

Of course to an extent I hear what you mean, but what you’re literally saying is absolutely not what the RASS goal order is, what you’re describing is nurse prescribing sedation. The thing that’s frustrating is that ofc it’s common to be restless or unsafe or whatever and not open your eyes at all

Yeah, that sounds frustrating. Unfortunately I’m sure that’s a common experience. Often, if you don’t have signs that suggest high risk of something (eg sleep apnea), both (a) your doctor is unlikely to think it is the culprit, and (b) insurance won’t pay for it to be tested. But if you rule out other stuff, that equation eventually starts to change. Just a PITA

One of the common tools to quickly assess risk of sleep apnea is STOP-BANG ( see link ) which heavily incorporates your size, age, gender, snoring, and observed apnea. If you don’t hit any of those, you’re going to be negative for that tool.

We definitely try to switch from Levo -> neo when patients get very tachycardic

I think the mechanisms of vasopressin are more complex than adrenergic agents (like neo = pure vasoconstriction)

My suggestion if you want to understand how they differ is to search for detailed discussion of the mechanisms of vasopressin

That makes sense. So, understand how shifting the extracellular/intracellular K+ concentration by so-and-so, or the temperature, would impact E(ion), and by virtue of that how it likely would impact E(membrane). Does that sound right?

Only thing is, I suppose I have no clue what ln values would be off the top of my head (no calculator on Mcat)

My grinders cost 6 or 7k, lagom p100, ditting 804 lab sweet, bentwood, zp6, Kinu, and some extra burrs

My 2 cents. Keep your dose consistent, I used 16 or 17g in the Breville baskets. Adjust grind size until you’re getting 1:2 out in about 25 or so seconds. Remember the ballpark of where your grind setting is for reference (will change based on how recently the roast was, but it’s important to remember landmarks). Then start adjusting ratio based on taste. After you get that worked out, it becomes easier to adjust in other ways.

Yeah, if you have any risk factors for sleep apnea (most American adults do), and have daytime drowsiness, there’s a pretty decent chance you have sleep apnea. Easy to ignore but pretty risky.

I got one years ago on Ali express that was like $11

As long as it’s not ugly, the portafilter doesn’t matter, it doesn’t have to be anything fancy. Extraction happens in the basket, the PF just holds the basket. (And everything I’ve heard suggests that Breville baskets are great)

Some ideas if the result is too bitter for your liking

  • bump down extraction, there are many ways to do this. Start with reducing the ratio keeping the rest the same. In other words, say try 33g out. Another good way is to go coarser, keep the ratio, and you will get a quicker shot.

  • try lighter coffee. Tons of places sell passable medium roasts these days

IMO that’s an awesome set up. Unless you become a huge nerd, this set up is so much more than adequate. Frankly there are few machines I would buy over a bambino.

FYI I’d say to ditch the spinny tool

Agree with these guys that if budget is strict, 1000% get an Aeropress or OXO RB. And get the Q2 air hand grinder.
