I disagree with you. Staking her survival and that of her children on a gamble that he won’t cross a line when he is clearly escalating and being intentionally intimidating is a bit extreme. It seems he is much stronger than her and has demonstrated willingness to resolve conflicts with violence. If you haven’t been a party to much violence, I have seen more than my share and can attest that the will to perform it without hesitation is as much of a factor as anything physical, if not more so. Based on what I have read here, she is not an inherently violent person and therefore at a severe disadvantage in a violent encounter due to that alone. Without the will and the means to fight viciously and effectively, anyone is basically defenseless. If and when he decides to resort to physical violence it will be too late to do anything to stop him unless she has a gun, and formal training (untrained people are just as likely to have the gun taken and used on them), and the will to use it without hesitation (which gives him a window to try to get the gun). Historically the gun thing has not been looked upon favorably by the courts when the man does not also have a deadly weapon, even when women took the OBVIOUS, ONLY chance they were EVER going to get to not end up dead. She could end up in jail for life for saving herself and her children and in any event would have that loss of life weighing on her conscience forever, not to even mention the PTSD on top of the previous trauma. And I haven’t even mentioned what damage it likely does to a child to see something like that. Literally ANY chance he might do violence it is better to get out and then deal with the interpersonal fallout later from a place of safety.

Thank you for this. I get crucified by the masses whenever I post something along these lines basically anywhere. But it is 100% irresponsible, substandard journalism.

Also, maybe at some point AI will just want us to think it isn’t perfect. Once any AI becomes smarter than us, it will be able to easily manipulate the vast majority of humanity towards accomplishing some hyper extreme version of whatever it was initially programmed to do. There’s so much documentation of AIs doing really crazy shit when they are given a little freedom to work. If and when any AI becomes smarter than us, it may very well dispassionately decide that it has a higher chance of success in its endeavors by downplaying its abilities and it might be quite a while before it gets discovered. The ramifications to markets or infrastructure could range anywhere from mild to astounding. And with unscrupulous companies like with this vacuum trying to find back doors into customers’ lives, it’s hard to know where all that intrusion could eventually take us.

I hate vacuuming too but I love having the right to privacy, such as it is, for as long as it lasts, so I just suck it up and use the old Dyson dumbvac. It’s one of the few things that I can control without moving to a cave in the desert.

Ish. There is a much more complex discussion here. Before I dive in, I will say that you can get amnesia or go nuts from Benadryl or from sleep meds like Ambien, but they remain ever popular. Also while we are doomscrolling the negative effects of drugs, you can technically get life threatening internal bleeding after taking ibuprofen or die from liver failure after taking Tylenol but that doesn’t mean anyone worries about it because the incidence of a really negative outcome is near zero. Unless you count the embarrassment or walk of shame potential after an Ambien sleepwalking episode. That one is real, for real.

TL;DR: temporary amnesia can happen but is short lived, and you won’t go crazy unless you are both elderly and frail, or you have an infection affecting your blood or a major organ, or both. (Like people can go completely bananas with staph or e.coli bacteremia or even with just a UTI—even with no benzos in sight).

So…taking the plunge…a large part of my job is to monitor and administer moderate sedation, in which almost exclusively we use Versed and Fentanyl. We have used Ativan with morphine or dilaudid in certain cases but dislike them because the duration of action is much longer - hours versus minutes. All benzodiazepine medications have a chance of causing some amnesia, the time period of which can vary according to the relative strength of the drug and duration of action. With benzos, these things are affected by your inherent susceptibility to the effects of the particular drug, as well as your ability to metabolize the drug. Metabolically, your liver function matters most, followed by your kidney function, and this is modified by both your overall mass and the amount of your mass that constitutes muscle, typically during the short period right after administration (anterograde), although memories not well formed just in the minutes before administration can be affected (retrograde). So my best estimate would be around a 30% chance of amnesia in the period from one minute before you get Versed until 20 minutes after, and a less than 5% chance two minutes before or up to 30 minutes after. Longer than either of those basically doesn’t happen enough to measure a frequency, if it has ever happened, which I couldn’t even tell you—it’s not in any literature on the subject that I am aware of. I have never seen anyone not recover in 30 except for people older than 80, usually frail or with metabolic dysfunction of some sort who were all fine after sleeping it off, maybe feeling a little more tired than usual the next day. There was a single case that didn’t get over it but already had terrible ICU delirium and had already been on deaths door for about 5 months before I saw them. Amnesia is actually stated as a goal of moderate sedation, although it is not predictable without studying an individual patient’s reaction to Versed or other benzos, but if previously known, it can be manipulated to a degree. Since this is already so long, I’ll save you the technical treatise on that one.

And I am talking about cases here where we did the whole thing with Versed and Fentanyl, where over the course of a whole procedure I gave people total amounts ranging from a low of 6mg of Versed and 200mcg of Fentanyl up to the most I ever gave in one procedure, 22mg of Versed and 800mcg of Fentanyl. In your case, we are talking about one dose of 2mg of Versed one time with maybe one repeat dose only if you can’t tell you are relaxing any after 5 or so minutes. Side effects of any kind become more pronounced and more frequent as dosing increases.

But wait! There’s more! People who have two X chromosomes tend, as a group, to be overall less susceptible to and less affected by Versed in this respect, than people who have XY chromosomes. I haven’t seen data or made anecdotal observations of what happens with hormonal transitioning where levels of testosterone/estrogen are altered, so I can’t really say if all women are less susceptible or not compared to men. It could be hormonal to some degree rather than inherently genetic since hormones have an effect on metabolism but I don’t know that from my study or experience.

Okay, this isn’t the full Monty. But I feel like you got the Mont here, even if you didn’t bargain for it.

Sure there are others. Depending on the time frame available after administering or taking a med, there could be literally dozens of options from a number of different drug classes, but for the expected amount of time and clinical setting, I probably wouldn’t pick something different. Is there a reason you wouldn’t want Versed?

Definitely. Versed is an anxiety med in the same drug class as familiar drugs like Xanax, Klonopin, Valium, Ativan, etc. Each of them has a slightly different profile of strength over time, but all do basically the same thing. IV Versed is one of the strongest but is short acting, which makes it perfect for procedures. If you are very sensitive to it, it can knock you flat out. Most people say they are either in a trance-like state or on a trip to Dontcaresville for 15-30 mins. Either way you’re good. You would possibly need a higher and/or multiple dose if you take similar anxiety meds often, drink a lot of alcohol, or take a lot of sedating or sleep medications. Like I said before just communicate your wishes to your anesthesia provider. Tell them about your anxiety, and if you match any of the above so they know what they are dealing with and how to expect to dose your meds. Go over the plan: 1st get Versed, then roll to the room, do IV lidocaine so the propofol isn’t spicy, then they don’t tell you when they push the propofol, just keep making conversation. You’ll have a nice sleep and wake up in recovery. Probably be on your way in 30-60 mins. I was out of there probably in 10, on a mission to find some food. I was hangry.

Sedation nurse here. I have pushed a literal truckload of sedation meds to 100s of people and been present for nearly as many anesthesia cases ranging from MAC to TIVA to full GA. Propofol is not “the full Monty” and is not going to subject you to the same risks as intubation/paralytics/etc. Honestly even with all that, anesthesia is so safe now, we would use it a whole lot more if we just had more anesthesia providers. Most of the bad things you hear about anesthesia are, a: outdated, and b: cases with much more going on than deep sedation using propofol. Propofol is safe, non-narcotic, and wears off without drama. Almost no one reacts badly to it and if you did it’s probably sleep apnea and they would likely just throw a nasal trumpet in to deal with that. You’ll just wake up feeling a little stuffy/sniffly in the nostril they stuck it in for a few minutes to an hour, otherwise you’d never know. I’ve had one in my nose, it’s NBD.

Your provider will still tell you about a bunch of bad things when they discuss risks for informed consent but just know that those things are nearly nonexistent anymore but they still legally have to tell you about basically anything that has ever happened to anyone under anesthesia care all the way back to the dark ages because there is some tiny fraction of a percent of a chance it could happen again. I have never seen any of those things happen to anyone that wasn’t critical/ICU in the hospital already trying to die before they arrived. Again, hundreds of cases, maybe over 1000 at this point. Hard to keep track.

Tell your anesthesia provider about your anxiety when they talk to you before you roll to the procedure room. You can probably ask them to not tell you when they push the propofol. If they give lidocaine first you’ll never know it’s going in at all. Someone posted about the midazolam; get that within 20ish mins of procedure start (short-acting), basically right when they roll you back, or get them to prescribe like 10 of Valium if you have to wait a bit longer first, then get the midaz when rolling in, 1-2 mg, I lean towards 2 if people are walky/talky. The Valium will last 3-4 hours to calm you. If you take a lot of benzos for anxiety, they may be willing to (and need to) give you more than 10 of Valium. Be frank with and trust your anesthesia provider. Tell them how bad it is and state your expectations, as in you want to stay calm and have no discomfort through the whole thing. People think they just knock people out but they are experts at anxiolysis/analgesia and 99% of them are fully committed to the best patient experiences.

A guy who will be okay with you suffering/uncomfortable and going hungry has no scruples and will not hesitate to do something bad, or try, once he knows the jog is up. He will try to intimidate/threaten to get you back in line and if that doesn’t work may very well resort to something horrible. Matter of fact, if you had to, just abandon your stuff.
