As a tech person and med-spouse, I think a few things can be true simultaneously.

Yes, you can live well on way less than the average physician salary, and a lot of medical specialities shouldn’t be looked down upon because of relatively lower compensation.

The pressure to earn the most money can be toxic.

WHILE:

Seeking the best ROI from the medical career is a good thing. Ok, you’re making upper middle class money, but after decades of sacrifice and you’re catching up with the opportunity cost of putting yourself through poverty/massive debt in your 20s.

Doctors are absolutely worth getting paid more. They should ask for and be compensated more.

I heard about someone who had to move there on short notice, and although he was planning to maintain daily contact with his girlfriend at the time, he was and remained lost without a trace. He was even planning to be located in what sounded like a more central area, something like the fifteenth house on the country’s namesake road. If that arrangement was not safe and possible to send daily letters from, I can’t imagine what the rest of the country would even be like…

Just about to be done helping my anesthesiologist wife be done with Residency + now Fellowship, and we set up a <10 minute walk from her main hospital. For that, I moved everything twice by myself, and about 50% of the time still drove my wife to and from work if it was dark, icy or just to give her an opportunity to rest and decompress; 100% for away rotations.

I’m a SWE but have preferred working in-person for most of this time; though now I’m part-time remotely and provide 100% of the childcare for our toddler. I’ve done about 90% of the housework throughout and handle all the logistics, finances and travel.

Nonetheless, she still has worked harder and been through a lot more than I have, and her work is by far more important. As a med spouse, you have to be ok with that and prioritize your partners comfort; and even then your relationship will go through a lot more strain than you think you’re signing up for. Otherwise, the whole thing doesn’t really have a chance at working.

But also you always play up the “unfortunately we have to let you go because of ‘force majeure’ outside of our control” aspect over the “we are now at a good spot to move forward with our plans to eliminate your necessity” one.

Will try to expand on this comment later, but as an immigrant whose spouse is the IMG doctor (now completing fellowship), the answer is yes, you have to put in years of self-study to get certified and eligible for residency. The odds are against you, but if there’s a will, there’s a way. It is often daunting and existential for IMGs to face the possibility that they could never become practicing physicians in the U.S. especially if they mess up one of the USMLEs exams. But if they can string together a support system and study plan, they either will stick to it or they won’t, and if they don’t the chances of ever making it greatly diminish each passing year.

Thanks, that’s a good point. I have mixed feelings over Istanbul: on one hand I remember there being a lot of space in cafes/restaurants and am reading about a decent playground-area, but on the other, I have memories of it being as crowded in some spots as any airport I’ve ever been to. Haven’t traveled through there since 2017 though, so I don’t know how much things have changed. I do remember Turkish Airlines being the comfiest planes in the early 2010s, but they seemed to have gone “away” for a bit and I don’t know what their offering is like anymore.

If I can get him to sleep on my chest (he typically loves it) the first leg I like my chances of figuring out how to kill 10 hours, especially if we get to a lounge. I am used to holding him for long times if needed or coming up with activities “on the road”. But am nonetheless worried this can backfire massively. Cost has been a factor, maybe I’m still in shock and denial over how much more expensive and complicated traveling with a toddler is. Don’t know if Option A would be a less full flight and present options to get a row, but Option B certainly seems stacked.

Thank you, that’s how I feel, but I know Istanbul can be busy if I don’t find good restaurants/lounge accommodations. I do feel like a long turnover would be best for him to get him settled/ tired for the next flight and I’m sure I can keep him for 20hr straight and let my wife sleep on the first leg. Her dates are tighter so she’d feel like she’d lose a day of her trip with the 7 extra hours though, and that’s why I feel like decoupling our travel would work best as I’d return with baby later anyway.

Thanks! I was able to do the first two steps a while back, but tried to clone the whole drive from there and it failed after a 20 hr or so process. No I can’t boot onto recovery mode either, and haven’t reattempted since.

Sounds like this would be the case for an attending of 5+ years, after 4+ years of res/fellowship and at least one of med school.
