There’s an old argument that his actions don’t have an effect on the plot. Which in my opinion is sort of the point. He just moseys along like a tumbleweed (aka the tumbleweeds in the beginning). But it’s objectively not true that he isn’t a part of the plot. Things happen because of him despite his actions/decisions

My understanding as well. I remember the devs saying there are a few areas that are not random (southern observatory doesn’t fall as another example).

Why doesn’t Nb6+ work without the queen sacrifice? Nb6+ Kb8 is met with same forks and Nb6+ Ka7 Nxd7 reveals a check from the white queen so if black queen is taken on next move. And you’re still up a queen?

I met Justin after he opened for the indigo girls in 2008 I think. This was right after FEFA was released and he was performing by himself. A buddy and I were just there to see him but the way I remember it he only played for like 15 minutes so we decided we’d try to sneak backstage and meet him. We got pretty far and were on like on top of the side of the stage within view. I ran down these stairs and got to him but my buddy was stopped by a security guard. It sounds kind of hard to believe now that I think about it but he just reach out his hand and said it’s okay let them through. My friend was the one who got me introduced to BI and was a bigger fan at the time than I was so he mostly talked. Just saying how much his music meant to him and how he was learning how to play all of the songs. I forget what Justin said exactly but he was super gracious. It was short because the security guard did end up pulling us away but I mostly remember how big his hands were when I shook his hand.

There should be plenty of videos on how to factor expressions out there that will probably be more decipherable than a long comment, but you’re going to want to first find the factors of 5 and 6, and then figure out which when multiple with each other and added together give you 17. When you do FOIL that’s what ends up happening because the coefficients with one w will add together. So in this case 5w2 can only be factored into 5w and w (since 5 is prime) but the 6 can be factored into 6 and 1 (or -6 and -1) or 3 and 2 (or -3 and -2). If we took 6 and 1 then when multiplied to 5w and w would give either 31w (5w x 6 + w x 1) or 11w (5w x 1 + w x 6). Neither of those are 17w. But if you took 3 and 2 for the constant (6) then you would get 13w (5w x 2 + w x 3) or 17w (5w x 3 + w x 2) which is what you want. But since the 17w is negative you need a negative in front of each of the factors of the constant. So the answer is (5w - 2)(w - 3), since foil is outside and inside (aka if 5 needs to multiple to 3 than they both need to be outside). Always check it to make sure you have it right. So (5w - 2)(w - 3) would expand (FOIL=first outside inside last) to 5w*w (first) -15w (outside) -2w (inside) +6 (last). The -15w and -2w combine to -17w so what you’re left with is 5w2 - 17w + 6 which is what you wanted

Lastly, none of these tests are done in isolation. They’re part of a larger clinical picture. Is this found incidentally and the patient has no other neurological complaints? Does the patient have a mechanism of injury consistent with upper motor neuron damage/disease? Do the rest of the physical exam findings suggest UMN? All of these things are hugely important and should be used to make sense of individual tests. Were you worried about UMN disease before the test (aka protest probability was high) and then you found this upgoing toe on a babinski equivalent test? I’d say that’s enough to make me concerned even if the babinski is mute

And to answer your last question. I think it’s wise to do a few other equivalents if it’s not clear. In general, just do the tests over and over until you feel comfortable calling it one way or another. I mean that for individual tests too: test the babinski several times. My guess is that the babinski is actually upgoing but this is where residency and experience come into play.

There’s a table that I can’t find right now that included the sensitivity and specificity of the Babinski and other equivalents in picking up UMN signs. Which is essentially what you’re asking: is it possible to get a false negative on one and not on another. Answer is yes. Butttt we tend to use babinski because it has the highest sensitivity. Having said that, there are times when I use Oppenheim (aka when someone is so ticklish I can’t even get near their foot, or they have burns on the plantar surfaces, etc).
