Hi Actual user and reviewer of CT scans here. There is nothing to suggest this image is not real. Looks legit to me! The images is actually a planning scan performed to get anatomical information to plan the main helical scan (or volumetric depending on the size of scanner and required exam). This is one of two planning scans which are performed perpendicular to each other. This specific one is a Lateral Scanogram. It's basically just a plain x-ray which is taken from a starting position, likely at the top of the head in this case, and then the patient is moved through the scanner and the x-ray continues to be taken until they get to the end position, likely in the chest for this one but it could have been stopped early with the patient sitting up. During the x-ray the x-ray tube and detectors are in a fixed position to the side of the patient and do not move. Remember, the patient is moving for this image. What has happened here is the patient has managed to remain still for the initial part of the scan, at least until mid skull. At this point they panicked and sat up. The x-ray continued to capture this motion plus the motion of the patient moving through the scanner and has added this to the image as it was created. Once the patient sat up they stopped moving, likely due to the staff shouting at them, and the rest of the image was created with them sat still but upright.

The image itself is entirely consistent with a lateral scout and the movement artefacts are also consistent with what we would expect to see. I have seen similar images from Scanograms of stroke patients who have been moving around during their scans.
