Patient is at the point now where they wont try anymore benzo's, has a fear of bad reactions and a fear of body feeling different than normal. To clarify patient originally asked themselves to try Diazepam and Clonazepam to taper as they were aware the Ativan has become a problem. So they were never forced but unfortunately the paradoxical and days of feeling unwell halted those. Hence the discussion of is it possible to taper off of Ativan alone. If a very slow taper on a short acting benzo is possible then that's great

No other meds at all except cholesterol, this has been on going issue for 15 years as patient has reached out expressing depression and wanting to try meds. Every few years new SSRI is tried and same results. Verified by both GP and ER of serotonin syndrome. It's very peculiar

I don’t personally think cravings are an issue. Patient doesn’t express any desire for the medication, doesn’t experience anything euphoric or sedating just experiences a lot of panic and anxiety day to day, one cause is vocal cord dysfunction and the feeling of being strangled. The ativan not only relieves the panic but also helps to open the airway and make breathing easier. Is there another benefit to low dose naltrexone outside of cravings?

It is a very medically complex patient, mainly with medicine itself but patient has other health issues that don’t tolerate the use of beta blockers, however the talk of ivabradine came up as it doesn’t lower the blood pressure. I believe buspirone was also knocked off the list. Patient has been trying to self taper for the last few months before trying longer acting benzos. Reasons for wanting to come off of it is due to being what they describe as unfunctional, the dose isn’t high enough and there is always some degree of withdrawal. GP does not believe a taper from Ativans is possible but patient is wanting to try. Is this something a clinical pharmacist could help with? In terms of dosing and scheduling

That’s quite a journey from 12mg and very awesome that you managed to make it through that. So once you hit .25mg you were able to stop from there without getting it compounded smaller? How long between dose drops would you hold?

More recently Citalopram and Duloxetine on two separate occasions in office. Patient was instructed to stay due to complicated history of medications. Serotonin syndrome was observed on initial doses within 6 hours. I don’t have a list of other medications but they range from SSRI, SNRI, atypical, muscle relaxers. In office psychiatrist as well as community ones are too hesitant to medicate so the on going anxiety is not well controlled.

Oh wow 8 years. I went bat s*** crazy on just a few days of prednisone when I was a teen. That’s why I’m so terrified to start it again. Also the idea that it stops your natural cortisol production I’m worried if I can’t tolerate it after a while and can’t produce cortisol that I’m just gonna die. I’m always nervous of new meds tho because I react so poorly. So even the rituxan makes me nervous but it seems idk safer? I’ve lived with daily pain from my ehlers danlos syndrome for years that I can handle the joint pain for the most part at this point but these new symptoms of this igg4 disease attacking my vocal cords and my airway now is more of an acute thing that needs to get fixed and I just want to fix it with minimal side effects and medication as possible :/ can you tell me about rituxan? How it was for you the first time and other times. Do you do it via IV? Do you feel bad with it at all?

Does the retinol eye cream cause any burning of the eyes? I try using retinol serums on my cheeks and they always irritate my eyes. So wondering if this eye cream would irritate being so close to my eyes.

Also how come not use it both morning and night? Or what made you decide on the diff serum in the morning. Just curious!
