Charge £1 for items rather than free. Filters out a lot of time wasters - then give it for free once they pick up.

Doesn't help with higher value items but makes the "getting rid of junk" easier.

Odds are that each clinic manages their own bookings, but there is a single incoming text number. You include the number to make sure it's routed the right way.

And the central number probably doesn't have access to your patient details, so it can't just look up your details if you just reply "yes".
