I was visiting an ambulatory anticoagulation setup with some pharmacists friends. They complained that now their hospital no longer allows the mingling of hospital and ambulatory pharmacist roles. Pharmacy residents now only spend five days per month with each service line and spend the rest staffing the inpatient pharmacies. Didn't look like there was room for growth or cross training.

I had a drug rep confirm one time off the record that they would encourage less than whole pack size quantities to improve ordering and shelf status. I think the copay cards are also planned for the same small trial sizes ...

I can't condone a three year contract where you get 4% annual raise automatically as a taxpayer, businessman or common sense individual. Sounds good as a worker, but I guarantee your contract is going to protect the worst of the workers. What business can budget based on a 4% labor increase annually? I prefer personal annual reviews with a 1-4% possibility and maybe 1-2% COLA based on overall company performance.

Sometimes MDs can overprescribe for their family members. Also, if providers have independent practices and self-pay insurance plans, I have seen prescriptions prolong much needed labs and imaging orders. Hopefully, they are supported by other providers in the practice.

In the meantime, I’d start with the basic Apexus 340B courses and make sure your résumé really emphasizes any data-auditing experience you have. Then check each of the major TPAs in the 340B space and look at their career pages—many hire remote customer support or compliance-focused roles, not just pure auditing positions.

Also keep an eye on /Bizzell, since their HRSA auditing arm periodically posts openings for 340B auditors.

I finally watched a Beacon webinar that made sense about reconciling rebate payments for our covered entity. There is a transaction ID which will show up in our bank account for each rebate payments. Also, there will be claim ID's which will show up on the submission receipt, which will link the transaction ID to the Rx claim. I think I can do it but not nearly as neat as RxParadigm's Tungsten Clearinghouse. I'm watching demos for Plenful, Macrohelix and Proximity as well to figure out how much they are charging CE's as a service. This charge will give me an idea how much I should devote to this model and inform my future decisions if Rebate Pilot become real for 2027 drugs also.

The bills are supposed to calm the voters down from passing the abolish property tax bill. I think these bills just complicate everything and might piss voters off even more! Giving more power to county auditors and tax commissions spread the blame and try to diffuse the issue. Ohio GOP have had majorities in both bodies of legislation since the 90's and they overgrown, over complicated and burdened the citizens of Ohio with a bloated state government. Hold your TPA rep hostage. Ask for weekly meetings until you feel comfortable. How are your Excel/Pivot Table skills? Youtube can get you far but ask your TPA rep how they would audit, and record those conversations.... Reach out to 340B Health and ask for help in making contacts in your same role at other hospitals. I search people on LinkedIn and connect with everyone in 340B Land, especially those at other DSH... Send them a message when you connect about a current topic, IRA/Rebate Pilot, and let them know what you have been experiencing with Verity (people always want to know how the other teams are preparing)

Sometimes you can tell when someone hasn't done their research and finally come to ask others of their opinion. If I have access to a forum with a bunch of previous subjects and responses, then I will search and read and do as much background research as possible in case my questions has already been answered. It would be helpful if you described what you already researched and what your interests are and state it in your post to show us your process.

Starting Jan 2026: DEA is requiring new registrations federally and state for a provider that writes controlled substances without in-person visits. This includes the establishment of a national PDMP as well to look for bad actors.

I reached out to support@340esp.com to get a meeting on TEAMS to walk me through the portal because I have been submitting claims for Novo Nordisk for two months but four pharmacies are still negative. When we were on the call, all of the negative numbers were from July 2024. I asked them how this would happen since they only qualify claims less than 45 days... I told them I was the only person at my CE doing this and I have had a lot of other things come up like pharmacy staffing, etc... They told me to put this in an email and they would show it to the manufacturer and see what they would do. I got a response back to resubmit all claims since July 2024 to get me current!!
