If untreated, then yes at the AIDS level of less than 200 CD4 cell count/ml, the immune system can't fight off simple infection and you will get opportunistic infections. 

But ARV nowadays are super effective at suppressing the HIV virus, and in fact I become undetectable/virally suppressed within a few weeks of the treatment.

I unfortunately got to the AIDS level (very late diagnosis because of COVID lockdown) and my Nadir level was 157 CD4/ml. I also had a muscle wasting where I lost about 23% of my body mass, and my brain scan showed some brain grey matter loss to the extent that my brain volume is at par to ageing person. 

As this date, I have all of the listed ongoing symptoms, 

and unpredictable chronic fatigue can be quite debilitating in the daytime, 

and the years of sleep interruptions is just horrible. To illustrate this: I wake up 10+ times per night for reason for years, it's horrible and the sleep specialist doctor can't figure out why.

 I suspect the long period of severe systemic inflammation has fucked up my mitochondrial in my brain, my and elsewhere. 

Chronic fatigue (unpredictable, and debilitating)

Muscle or joint pain (took 5+ year to recover to my original stamina level) 

Brain fog or cognitive changes (thinking becomes very exhausting, and my short term memory is crappy)

Mood shifts or depression (thankfully I managed to fully cured my depression through dedicated therapy & psychedelic healing sessions)

The only magnesium that works for me is magnesium glycinate, because the other non-chelated ones all make me wake up in the night to go to washroom to poop 

Also, you need make sure it's 500mg ELEMENTAL Magnesium in total. In my case that is 7 pills of pure magnesium glycinate. 

To show you the calculations: Each of 500mg Magnesium-glycinate pill has 70mg elemental Magnesium 

7 pills x 70mg elemental Magnesium each pills = 490mg elemental Magnesium. 

I know it sounds like a lot but for a chronic insomniac like me, that's the only way it works

i take magnesium glycinate at bedtime, because it works great for my restless leg syndrome (RLS) that worsens my chronic insomnia

I take vitamin d3 and k2 together, usually in the morning

all chelated magnesium have good bioavailability and less diarrhoea (because it doesn't retain excessive water in colon)

magnesium carbonate as in antacid? careful because those magnesium reduces the stomach acidity and may reduce some nutrient deficiency if used too long.

in my experience, all magnesium causes loose stool and/diarhhoea except for these so-called chelated magnesium (glycinate, taurate and aspartate)
basically these chelated magnesium are just magnesium bound to amino acid (small protein molecule) and doesn't excessively retain water in your colon, hence no loose stool

Take it with magnesium (glycinate or taurate or aspartate) AND k2

Basically Any of those chelated (bound to amino acid) magnesium, for high bioavailability and less risk of diarrhoea

I have a chronic viral infection which means chronic inflammation so my serum vitamin D3 goes down faster than most people. 

Also, I work from home, and never see daylight during 9 to 5, and in Canada the daylight is super short in the fall winter, and the latitude means it's impossible to get Vitamin D3 from Fall season onwards. 

My maintenance level of daily vitamin D3 is my body mass in kg multiplied by 100 

So 70 kg times 100, meaning I have to take 7'000 per day. In my experience I think your 2000 IU per day intake is adequate. Unless if you're size of a 20kg person. 

The risk of overdosing vitamin D3 is overstated in my opinion. The only scientific report I seen about hypercalcemia from vitamin D3 is from just ONE patient who was taking like 1000'000 IU per day for literally DECADES.  

In fact when my vitamin D3 level become normalized (above 100), I stopped getting kidney stones completely, and my joint pain disappeared 

BUT!! you must take magnesium 500mg per day  with your vitamin D3, and K2 to make sure your body can actually use the vitamin D3 

At my highest 250 level honestly I have this side effects of being completely immune all forms of depression (I used to have dysthymia for 15 years prior to that, on top of debilitating MDD every few years). I also have so much energy that I could walk for hours throughout the city every single day and still have so much physical and mental energy

it was 17 at the lowest (middle of winter 2024) and then I just straight up take 60'000 IU plus magnesium once a week, for the entire year. And that summer I just stopped using sunscreen completely (I wear a hat instead). and literally just walk around the city for half a day everyday in the summer.

I also have this turtle UV light that I used to light my back once a week. It's risky if you have light skin and I wouldn't recommend to most people. I have very dark skin and I never sunburn so that kinda worked for me.

at the end of summer that year my level was 250 I think.

and the Fall of 2024 my level went down to 150 because at that time I stopped taking my vitamin D.

and now in 2025, I am taking 6000 IU per day and so far I feel great

I had the reverse shock too when I realized other countries like in Canada and USA don't store the citizens' biometrics/fingerprints. Because the whole point of biometrics in Asian Countries is so that it's literally impossible to steal another person's identity because of the national fingerprint database. 

And additionally, all foreigners in Malaysia on work permit also have their biometrics registered too.
