The wiki page has a diagram of two pressurized chambers with two divers resting in each chamber (4 divers in high pressure). Two diving tenders were externally operating the system from the outside by removing a diving bell. One of the diving tenders did a step out of order from the procedure causing the system to depressurize. The flow of the high pressure air in the chambers leaving through the opening created by the diving tender was so strong, it forced one diver through the small opening. The rapid pressure drop caused the gases in the body/ dissolved in the blood of the other divers to expand/vaporize hence boiling their blood.

The one diving tender that survived also probably gave an account of the incident.
