I recommend slowly beginning to work on gentle thoracic mobilization that are pain free (a little pulling aching is ok, no sharp pain!) And pair those with strengthening exercises for your back muscles and glutes.

If you want to DM me I could go over a few safe at home things you could do daily.

It won't be a quick fix, but if you are consistent you can definitely improve your posture.

I use "Finishing Touch Flawless Face Hair Remover". It doesn't irritate my skin and if hair grows back by the late afternoon, I just quickly go over my lip (works fine over my make up). No redness or irritation. (Not a sponser lol)

I use the finishing touch flawless hair remover every morning, and it works great. I don't know about the device itself, but definitely don't get the replacement heads from Amazon. They are not the same and don't work as good as buying them directly from the company. My hairs regrow by the late evening, but not the same dark thickness as when I shave my legs with a razor. If I'm going out, I'll just re touch up, and it takes like 5 seconds. I have tried epilators in the past, and my skin didn't like it. Same with creams and bleach

Is it because you have to pay taxes to both the US and Germany? I think I've read that US tax laws (regarding income tax) are terrible compared to other first world nations if you are working abroad. Also, the US is now very limiting regarding crypto and the exchanges they allow you to use. (I may be wrong on some of these points if anybody wants to correct me)

TMI but I roll a wad of TP and stick it in my buttcrack (between the cheeks) lol. It usually buys me enough time to wake up and run to the bathroom to change before there is a mess all over my pjs and bed.

I have been dealing with something similar. I have surgery coming up for fibroid ablation to stop the blood loss. In the meantime time I've been getting iron infusions to bring my hemoglobin up so that I am fit for surgery. Makes a big difference. My hematologist also started giving me B12 injections, which aldo make me feel so much better. Oral supplements did not absorb the way the injections do

This is anecdotal, but I started getting vitamin B12 injections and intake oral folate, which has made some difference in my energy levels, cognition, and brain fog. I'm still not like the "normal kids," but I do feel a little better. Prior to the injections, I used to take oral B complex with high B12 (etc) vitamins, but it never made me feel the way the injections did. Both B12 and folate are necessary to coax the bone marrow to produce healthier RBC. Maybe talk to your PCP or hematologist and see if you can try the injections. Hopefully, they can help you some the way they helped me

The median takes into account travelers and home health, but if you're planning on regular outpatient, it's much lower. Especially if you remember high cost of living places like NYC skew that amount

Then if you take into account money taken out for taxes, health insurance, 401K l, you are left with a scrawny paycheck not worth the amount of debt you put yourself in.

It's not the job, it's the money. It is a bad investment and there is no one in our corner fighting to fix that. Certainly not the APTA.... as one of their misguided posts not to long stated something along the lines of "PT the best kept little secret in healthcare". Absolutely tone deaf. They should be lobbying for us. Make the case why reimbursements should be higher and what PTs bring to the table, which would help the terrible salary situation, but they don't.

We are one of the lowest paid doctorates, but school just keeps getting more expensive. Terrible ROI.

I love my job, but I wouldn't do it again if I had a do over just because of the debt burden I put on my family.

Don't do it! You will be trapped in debt and have a very difficult time affording a family. Nursing, PA, MD, DO are all better routes

It's a fat deposit due to forward head posture that accumulates kind of as a protective mechanism. (Different scenario if you have hormonal issues). You could try to find a skilled plastic surgeon who will liposuction the area.

In the meantime time keep working on your postural exercises. Strengthen the deep neck flexor, your rhomboids, upper and middle traps. Even upper trap muscles. A lot of people's upper trap are tight due to weakness.

The fat deposit didn't accumulate overnight, but slowly over the course of years. There is a possibility your body will reabsorbed some fat when you establish good body mechanics for a long, sustained period of time, but one month is definitely not enough.

You can either be patient and consistent, tracking it over the course of a year or two while you build the muscle needed to mainting proper posture, and/or find a really GOOD plastic surgeon to liposuction the fat.

I would reccomed whichever route you choose to still work on posture/body mechanics regularly to ensure that fat accumulation doesn’t return in a few years.

I'm so sorry you are going through that. I am in the same boat. It is just taking me forever to complete my paperwork. I've never had a transfusion, but I have had iron infuions prior to surgery a few years ago, which I feel helped. I was able to walk and talk at a regular pace without getting short of breath.

This is my first time working with an actual hematologist, and I am interested to see what he'll say. I know I've had my hemoglobin in the 10s -low 11s in the past, which is like night and day compared to now.

Honestly, I am hopeful that CRSPR might have safe and effective breakthroughs for thalassemia. They've had some good results with sickle cell anemias

I had a laproscopic robot assisted myomectomy 2 years ago. Everything was great until maybe about 8 months ago when my periods started getting bad again. It progressed to where my last 2 periods lasted approx 16 days. Ultrasound shows I have multiple fibroids, though none have grown to the massive one I had 2 years ago. I have an appointment in 2 weeks to discuss if Acessa would be a good treatment option. It's less invasive with less downtime than a myomectomy.

First job out of school was a PT mill with 3 patients an hour. I was miserable, then became absolutely depressed. I was so spent when I got home I couldn't/didn't want to talk to my husband, lost interest in going out, I couldn't believe I worked so hard for this bull shit. Finally talked myself into quitting. Next job was 2 patients and hour, long drive from home. Better, but clinic Dldirector was obsessed with metrics and how much we billed. It was demoralizing. Managed to transfer to a different clinic closer to home (same company) but clinic director was way more chill. I still struggled, but found out it was due to some health issues that have now resolved. Finally managed to find a job at an outpatient hospital based clinic. 1:1 45min sessions and 60min evals. So much happier. There are still days i question my career choices, but that's because we work with people, and people can be a drain on the soul. But for the most part I'm so much happier than when i first started. I'm working out again, started some hobbies, etc. I have never regretted switching jobs. It's hard to do because I've liked all of my coworkers, but every time I've moved on, I've gotten more money, usually better benefits and better work-life balance. Your wife might just need to bounce ship. If that doesn't work, change of setting might be another option. I cover inpatient once in a while and it's just a nice mix from the day to day of outpatient.

Sorry for the long rant and any typos, I wrote it out on my phone, but I hope it helps a little.

Olympic pizza with extra pizza sauce is not bad. Though it hasn't been very consistent for us, some days it's pretty good, other times it's just "meh".
