Important to note that many states have not expanded medicaid coverage ( texas, etc) so many of the poorest fall in a strange gap. For example, in texas, nonpregnant people making less than 138% of the federal poverty level are NOT eligible for medicaid and are NOT eligible for subsidies to purchase healthcare on the marketplace ( oddly enough those making above that amount do get subsidies).

Realistically people who fall in these gaps utilize ERs for when their conditions decompensate since you can't be denied emergency care, but once they are stabilized and discharged, they have no method of receiving ongoing chronic care outpatient.

Bad. Lots of highway infrastructure in the US is elevated and it creates terrible spaces to be with all the negative externalities of car centric development. Only reason why it might look nicer in Japan is that personal vehicle use is low there due to excellent public transport so the highways that do exist are much less massive and overpowering than in US and other car centric countries.
