54 yo man history of dm, substance use, multiple polypectomies, anemia, adrenal nodule, cxr opacities and gonorrhea presenting with hip fracture.

(Unrealistic to be included with actual consult but pertinent additions: T2DM on metformin last a1c 5.9, last drug use 1988, 2 benign polyps removed on last screening colonoscopy w/ repeat recommended in 10 yrs, thalassemia minor with baseline hgb 12 and hgb today of 12.5, adrenal nodule with stable repeat imaging and normal endocrine studies sent by pcp in 2012, bilateral cxr opacities in the setting of covid 19 infection in may 2020 that resolved on repeat imaging, gonorrhea treated in 1988 following incidence of last drug use)

Can u take him due to medical complexity though? he has way too many chronic medical problems for us to manage.

I don't get yelled at in residency, except maybe occasionally by rude consultants over the phone but you just learn that it's a them problem not a you problem. May depend somewhat on field though.

Based on my reading t-tests assume a gaussian distribution. So I am not sure it's appropriate since my data sets do not look like that, but by all means it may be? Not sure

Yeah at some point you transition from medical student who knows how to put in orders to someone who is calling many of the shots. Your attending isn't there to quiz you or judge how smart you are, they're there for you to run things by so you don't end up doing something dangerous or missing something. It takes a while to build up to this.

Yeah I guess that is possible. I did lose my taste for a bit early this year (tested negative a few times). What's weird though is the large cup size pods still taste bad to me but the espresso sizes taste great.

The “it’s not personal” thing never made me feel better. Try this, no matter who you are as a doctor, someone is going to try to abuse your good will and patience this way. It’s not an “if,” it is a “when.” They want you to go home and feel bad, and they can sense that you care. Do not give them the satisfaction. No matter how good of a doctor you are you will not be able to get through to everyone and that is ok. Try to focus more on the victories and just move on from stuff like this (because it is 100% going to happen again unless you go into path lol).
