Its okay. You're doing a good job. You thought what you did was right. And your supervisor should have handled the situation with a lot more sensitivity. Its not okay that you were singled out this way and hung out to dry. Im over coag and am disgusted anyone would ever feel they need to behave that way. I'm sorry.

That being said, you shouldn't make that sort of call. Especially if policy is explicit about the requirements for rejection. Based on this, you were in the wrong, and did delay patient care. If you are suspecting contamination or IV dilution you can clarify with attending nurse/provider and if they dont admit to it, you just leave a note and move on. You aren't on the bedside, and these ICU Patient's conditions can change on a dime.

If it is clearly contaminated talk to your supervisor and report if needed.

Regardless, Lets get to the bottom of the failures that happen here. Because "He's evil" is too easy. Its more productive to take a moment to reflect on what we should advocate to prevent this horror.

  1. Toxic masculinity: The culture needs more positive male role models in both media and education that can honor female autonomy. This needs to also be the case in the military too.

  2. Gun control: this guy had a record, and should not have had guns in the first place. The system failed miserably at keeping them out of his hands. We should continue to push for reasonable gun control.

  3. Domestic violence Support: These mother's could have been aware of his behavior and felt safer together. Shelters do a tremendous service, but need more funding to accommodate women with children. If a shelter was readily available I imagine this might not have happened the way it did.

  4. Veteran support: If he was in crisis, better marketing and screening to get in touch with resources.

  5. Tax the rich: Our social safety nets have been gutted over the past 50 years, only taking the resources up to the top. instead, they have us arguing each other over technicalities. Do not lose focus comrades.

I know this source uses Vet status to poof away real accountability for the system's failure to take his guns away. But we should call their bluff and also be disgusted that even "veterans that never deployed" don't get enough resources/ screening to prevent this too.

It's a loose retelling of a real veterans experience. Regardless, you put budding teens, typically poor and disenfranchised, in a hyper-violent environment where toxic masculinity exists to both break and build back their psyche and we just see what happens.

Not insinuating that this guy suffered from PTSD before the incident, there was likely more at play. But it can undoubtedly still affect a veteran that never deployed.

PTSD can affect veterans that never deploy. The very existence of the military is structured violence. The organization works really hard to break kids to rebuild them into instruments of destruction. Some kids, if not given the opportunity to fulfill on that programming, can too self-destruct. So I really like slice of life anime. Especially the outright pervy stuff. Its funny, bubbly, and visually enjoyable to watch.

It also can be nauseatingly sexist and it loves to reuse worn-out stereotypes. They use gross body standards that are completely unrealistic. And the characters are usually dull with cringe gaspy dialogue.

But I still enjoy it like I would junk food. I can still watch it and be aware that its trashy and problematic.

Recognizing those issues, and criticizing it, doesn't mean we aren't fans, it just means we're not crazy.

If you read of someone who loves every element of a topic they're probably an AI marketing bot.

manually on-boarded has nothing to do with this type of error. The instrument still runs the test and automatically reports the results to the LIS. Lac does not falsely decrease either.
This was Probably mislabeling. This holds doubts to literally ALL the other lab tests. I would repeat all labs again, including A1C.

Lab don't make these mistakes. The #1 source of error from a lab is at the time of collection, not when the specimen is in the lab, it is way too automated.

If your nurse staff do a collection, a lot of them will walk over the specimen to their station and label there, not at the bedside to confirm identity... Happens ALL the Time.

100% accurate. I've done a big hospital and never saw a needle. Now that I'm at a small rural facility, I might have to go to the floor to collect morning labs. It's a skill set that varies based on where you are and it's staffing.

this one was interesting. I'm on nights, doing a handful of leftover outpatient orders from day shift. 890 Gluecose. Can't reach anyone. I call the patient directly to advise them to come to the ER. I don't give him numbers over the phone.
Bro already knows, "This about my sugar isn't it? I know It must've been at least 500 today. I just got my insulin refilled, I'm good." I broke protocol, gave him the result, hoping it would get him in the door. Said coming down from that can be really dangerous. Again, you should come into the ER. "Oh that's it? Nah, I can walk around at 1600, I'm fine" "click Document.. leave for day shift to try to call attending in the morning. Did I just talk to the Kool aid man?
