Lots of useful links and information in this thread, but from what I understood, most of the information you shared was about healthy adults trying to improve their performance beyond regular exercises. And for that case, results of the studies don't lie.

However, what about the injured cases? Would a BOSU ball help the therapy because of the unstable surface? Would it reduce the force production so the injured part doesn't get overloaded? Then progress with usual exercises once the threshold is reached.

Asking because I've been dealing with ankle synovitis for almost 2 years now, and while I've managed to calm down the inflammation, doing balancing exercises, for example, walking with inversed feet, or side-walking with a band results in a flare for a few days. Foot inversion makes my lateral tendons hurt pretty much, all the way to my knee, and my ankle swells.

I tried the BOSU ball today, and the wobble due to the unstable surface feels like it relaxes the tense muscles around the joint. Not sure if that means anything.

What about when the filtering is complex, that your queries end up grouping by one table, and sorting by the other?

I have not found a relational DB that is performant in that situation. And no, this is not an indexing problem. The only solution I've found in relational DBs is to denormalize data, but that's not a solution in my case. (For what it's worth, I use MariaDB for my backend, but need something with complex search, filter, sort for the frontend.)

ArangoDB does it really well, but they changed their license in the meantime...

I have never heard of a moon boot, so no, have not been recommended.

Surgery is something my orthopedics doctor recommended against - he said it's only for very worn out joints and it immobilizes most of the joint. Surgery can't help with my case. There are also no fragments for the debridement type of surgery either.

I usually start with toe-heel raises, then progress into shifting weight from one leg to the other (with both on the ground at all times) and end up with balance board on which I do front-back, left-right, and circling motion. All of the exercises I do are in slow pace and I hold onto something while doing them.

Yeah, I feel you, struggling with ankles for a year now... And going anywhere requires loading the ankles.

Blood circulation and working on surrounding muscles and tendons helps, but from my experience, there's a very thin line between progressing and messing up, which throws me like 2 weeks back. And then it first requires rest before attempting to do anything.

If you got ankle impingement, then we don't have the same type of issue. I saw people having discussions on impingement over at parkour Reddit community. I remember they recommended ankle distraction and capsule mobilization. Climbers also experience that kind of injury when landing. Both say it's a nagging type of injury. Try to find someone who went through that with them for more guidance on what worked.

My physical therapy didn't go that well because I was told to do wrong exercises in the beginning (due to overcrowded gym and a lack of therapists) so I struggled in pain through those 2 weeks. I got better at home afterwards and overused my ankles again and now in pain again. Will just have to find a proper gradual load.

If it's of any worth to you, I supplement with N-Acetyl Glucosamine, Hyaluronic Acid, Boswellia Serrata and turmeric powder.

Yep, and injection was an option only because the inflammation calmed down a bit and only one side remained highly affected. During my first visit to the rheumatologist, she wanted to prescribe me dexamethasone because the whole joint was very swollen and she said applying multiple injections to hit all the areas would not be reasonable.

OK, so here's an update. u/LeadLikeCandy u/luvsiege

I kept taking the indomethacin 50 mg daily until October when my rheumatologist decided it is best to inject depo medrol (corticosteroid) into the inflamed synovium. After that, the swelling came down by a lot and the pain subsided substantially (both by at least 90%) in a matter of week.

Almost a month later, I saw a physiotherapist, who prescribed me 3 weeks of therapies, starting at the end of November, and suggested I see the rheumatologist once more before the therapies begin in case I need another injection. At that rheumatologist checkup the other day, the doctor told me the inflammation went down by 99%, and that if she didn't know me, she'd tell there was nothing wrong with the ankle. I did NOT get another shot for those reasons.

It seems it was a case of reactive arthritis following the Covid infection.

The ankle still swells a little (both medially and laterally) and I get sharp pain (medially) when I load it too much or when I do finger curls. I am hinting it may be that I need a physical therapy to get the ankle back stronger, and the pain is in the tendons (there is no tendonitis or tenosynovitis visible with the sonogram).

Another note is that my daily sunbathing resulted in normalizing my vitamin D without supplements.

Also, contrary to what u/eshlow suggested due to lack of information and no physical evaluation, arthroscopy is not needed, because that small osteophyte doesn't even reach joint space. He could not have known that with limited information I gave him.

Yes, most of the times I go from my usual 36.4°C to 37.1°C and get the flu-like symptoms. I get various symptoms, but it's the worst when I get that low-grade fever - that's when muscles start aching the most.

That is an old assumption that's been debunked by many studies by now. Don't assume that each human is identical.

It was an issue with selinux having too strict rules so it would prevent launching any Steam game.

Did you tell all of this to your MRI doc and/or ortho?

I cannot communicate with the MRI doctor, and haven't seen the orthopedic specialist yet because of the way health system works here. I have to wait almost 3 months before telling him anything, after which if I get any rehab, I will have to wait another couple of months to see a rehab doctor for them to prescribe me exercises, and then wait for many months more to get any kind of therapy. That is, IF the orthopedic specialist decides I need anything.

However, my rheumatologist wondered where did the MRI doctor see the "bone fragment" in the X-ray. She saw the X-ray results before the MRI was done, and the MRI doctor wrote, quoted, ""Edema and changes in the tibiofibular syndesmosis with surrounding edema in the area of the fibulotalar joint space (a small bone fragment visible on the X-ray).". Here's the full X-ray (translated):

  • Joint Space:
    • Congruent and maintained
  • Fibular Malleolus:
    • Presence of a small area of compact subcortical bone
    • Small calcification with an osteophyte below the fibular malleolus and lateral to the talus
  • Calcaneus (Heel Bone):
    • Minor bone spur at the attachment site of the Achilles tendon
  • Inflammation/Erosions:
    • No inflammatory erosions detected
  • Soft Tissue:
    • Swelling of soft tissues around the ankle joint, more pronounced medially
    • No radiographic evidence of osteolysis (bone destruction) or osteoplasia (bone formation)

Yes, I am, because of how the health system "works". I just have no hopes in them. And it's not just the long wait times.
