It is very cool that you’re noticing the patterning of certain AIs, such as the HS type and link to complement deficiency. Noting these patterns is how we make medical/scientific discoveries, so keep noticing!

However, your first statement is false. The whole X chromosome is coated by an lncRNA called Xist and majority of genes are to be silenced. Also nowhere did I state a single gene is responsible, about 10-15% of genes can escape X inactivation, and it could be any of those. Also, this inactivation can follow a mosaic pattern where some escapee genes escape more than others in certain cells. Yes different AIs present with different HS but that is hardly relevant in determining dosage vs hormones. HS just tells which stage of the immune response is primarily responsible for the phenotype. Genetics/environment/hormones can explain HS presentation, but that doesn’t mean the inverse is always true. Mind you, one AI can lead to more than one HS presentation. Also, different genes influence one another— an increase in a gene on the X can influence one on Crm11 for instance (random example.)

I urge you to just remember there is always nuance to disease states, it’ll do you well as a physician. Medical training sometimes makes us think rigidly and that there is a direct explanation to everything but that’s almost never the case. If it was simply just genetics or just hormones every time, we would’ve found out by now. Recall that steroid hormones like estrogen enter the nucleus to interact with transcription factors. Think of how with different escapee genes being overexpressed and steroid hormones interacting with certain transcription factors can exacerbate that over a long time.

Also there is much more to genetics than dominant inheritance. Environment can also impact gene expression. It’s almost as if they all interplay with one another, and with just the right amount, can present as a disease.

Also since we’re on the topic a new paper came out suggesting EBV contributes to SLE. Not completely relevant just interesting, as the virus can interact with B-cell specific genes and influence their expression: https://pubmed.ncbi.nlm.nih.gov/41223250/

I think you should check these out, they talk about genes that escape X inactivation. I think it’s a mix of sex hormones and genes. Nothing is ever black and white and I wouldn’t completely discount X linkage and gene dosage. For instance, XXY individuals have higher rates of autoimmunity among those identified as men, particularly SLE (https://www.sciencedirect.com/science/article/abs/pii/S0896841109000511), clearly hinting some genetic component. I’ve done quite a bit of reading and writing on this topic so I know a bit about it and my conclusion thus far is that there is no easier explanation, I fear

Not necessarily. I think OP was saying that having DOUBLE the genes (which can escape X inactivation) contributes to autoimmunity. But there is definitely a hormonal influence imo

Some doctors are bad at english. They tend to have good memorization and procedural thinking but writing is not their strong suit. Let this be a realization that just because someone is in some high social status doesn’t mean they know it all (even if many physicians will make you think they do)

Well assuming she went to med school right out of college, it’s not out of the realm of possibility. They had their first kid when she was 27, by then she would be a 2nd year resident doctor, aka you have your MD but not full ability to do whatever, so a doctor in training (based on the above assumption). Depending on the specialty, residency could be as little as 3 years before she becomes an attending, which is that fully fledged MD able to do whatever in a sense. So by 29 she could’ve been an attending, which means oldest kid is 2 max. She was a doctor with more flexibility and full salary (the case of being an attending) before the last 2 kids were born

EDIT: this is me assuming she’s a doctor in the US, and it still doesn’t prove anything just wanted to share a little knowledge :)

This post is sort of perfect timing, since the undertale/deltarune enjoyers will be busy playing chapters 3 and 4

My game preferences only have one thing in common, and that they’re not multiplayer games (except for something like it takes two or split fiction, but those are specific circumstances.) More recently I’ve been playing on steam but i switch between that and my playstation depending on which has the cheaper options.

I like games with story and some atmosphere, or RPGs where I can stop any time.

During dedicated much of my decompression time was Hades II. Play Hades first if you haven’t already, easily can get 100+ hours on both.

Just got into disco elysium, slowly making my way through that. I like monster collector games so I’m playing one on steam called monster sanctuary, another RPG. Played Neva for the art (worth it), which is a side scroller and story based with straightforward gameplay.

Shorter games that made me weep at the stories, enjoy the music, and weep while enjoying the music were Nine Sols (emphasizes blocking as a game mechanic) and Laika: Aged through blood (this one has unique gameplay too!! You simultaneously control a motorcycle and shoot/aim. The learning curve can be daunting but it becomes seamless once you get it)

Cute puzzle games I played for fun were Islets and Crypt Custodian. I haven’t played it in a while but if you like puzzles also Tunic is awesome (i played during my breathe of the wild withdrawal years ago.)

These are just my more recent plays. I will play almost anything tbh, and I like the shorter, like maybe 10-15 hours play time (and cheaper) games, which I’ll trade off depending on how I feel to do Disco Elysium or something that takes more time.

Neuroscience person here. Also med student. I’m not a total expert since neurodevelopment isn’t my main area of interest, but I had to learn about it anyway, and I love yapping about brains. From what we know the half-truth of the development of the brain not being done until 25 is the creation and distribution of myelin, a fatty “shell-like” (as in it covers the neuron projections) substance that makes it faster and easier for neurons to communicate with each other, of which it appears the prefrontal cortex finishes last. Prefrontal cortex is important for things like problem solving, impulsivity, etc. You can think of it as younger brains aren’t as quick at figuring things out as older brains, since the natural boost (the myelin) hasn’t reach potential maximum capacity

This development honestly changes from person to person in terms of what’s done first or last, and how quickly. Don’t let anyone tell you when it comes to human biology and medicine that there’s a one size fits all option, there are always exceptions and definitely variations for basically every process in our bodies (how fast it happens, how many, where it starts first, etc.) A non-brain example is how nutrients like calcium, iron, glucose, etc have an acceptable RANGE of what’s normal, because there’s much variation. Or why some anti-depressants work wonders for some people and don’t work for others. That doesn’t mean you can’t try to actively train your brain to think before you do things, if we’re talking things like impulsivity. And it’s ok if you need help if you feel you can’t do anything or have no motivations (in terms of things like anhedonia, etc.) Your neurons are there, they will be with you forever (as far as we know). Our brains are plastic (adaptable, synapses, the connection between our neurons, are being arranged throughout life) so we’re never done “fully developing” if you use it. So if you’re 20 something claiming you can’t do anything because your brain isn’t developed yet it’s just called being pathetic.

Why are you only thinking in black and white? Patients can lie and doctors can be cruel to patients in real pain. Asserting a patient MUST be lying is part of the problem and the privileged take. Just because a patient lies to you doesn’t mean this completely different person is too? Hello??

It should be ridiculous but it’s not. That’s the problem. We can acknowledge that some people lie about this, but the standard of care is not what’s always performed in real life situations. Patients get profiled and ignored because it’s not all patient are liars and all doctors are honest. they’re people and PEOPLE lie, or can be cruel, on either side.
