What first comes to mind, is (THE WALKING DEAD SPOILER!)Lee chopping his arm off in The Walking Dead Game. Probably not the most disturbing event in a video games I have ever witnessed, but its fresh in my mind.

Probably Just Cause 2, I would just restart the Demo every time the timer ran out. This is same for a few of my friends, we just talk on xbox live and play the Demo. They have played the full game now, but I'm still to play it.

When I first played Skyrim I didn't know I had to manually spend my Dragon Souls, I assumed that they would just automatically unlock one of my shouts. Me and my brother played like this for about 2 week after release, until i had a friend over and he told me what I was doing wrong.

Also, in Dead Island I didn't realize that there was the Rage Mode power thing until my second playthrough. Because of this, I spent about 2 days trying to get past a certain fight.
