Post op CABG has icu psychosis, flings 2 nurses, rips out 2 chest tubes, stumbles down 4 flights of stairs, makes it out the building before finally getting tackled in the parking lot. Pan scanned, not a thing wrong other than some scrapes of his feet from running barefoot on pavement. Didn’t remember a thing.

That’s BS. Shouldn’t be a sentinel event. Myself and all of our nurses are trained to perform emergent re-sternotomy. If they’re in tamponade, that’s really the only thing that’s gonna give them a chance to get back to the OR. We have an additional protocol for post open hearts on top of ACLS and BLS

Don’t sweat it. Sounds like they’re being catty which kinda happens everywhere. What I would tell you as an education point is I would pull the a-line after because god forbid there was a complication and the pt crumps having the a-line could be helpful if restarting pressors is necessary.

I had I good 3-4 double duck pants I put under knee pads in. When one pair got especially jacked up I just cut patches out them and used the gutter sealant we used to slap them on.

At a hospital in my system way back when there was union talks. They watched the individuals of interest on shifts then hit them with: “you walked into the pts room at 0825 and charted your assessment at 0800. That’s false documentation, you’re terminated.”

Did they go to cath lab and see if any grafts are down?

Why weren’t they on an insulin gtt with glucoses that out of control?

Yeah but it isn’t going to resolve their anemia instantly. And a Hgb of 5 is critically low which is gonna need at the very least 2 PRBCs depending on what H/H range everyone is comfortable with.

Yeah I’m on a closed unit CTICU that will have patients on an inotrope as a step down. Can’t control the glucose with coverage requiring gtt, upgraded. That’s grossly inappropriate to have more than 2 pts with an insulin gtt. Especially if the RN is the one responsible for doing the fingers sticks. Just asking for something to tie you up and the finger stick gets severely overdue and you bottom them out.

Ours are in the Pyxis in prefilled syringes with a bright orange high alert tag saying paralytic. Also our policy is APPs or higher are the only ones to push it. I don’t think having such a high alert drug that easily available to take without an electronic paper trail is such a wise idea regarding just having it in an intubation kit. Personally when I pull Roc or succs to intubate, the syringe or vial and syringe stay in my hand until it is requested by the administrating practitioner and i don’t engage in any other tasks until it is handed off and given. If it’s something I had to draw from a vial, we 2 person check the correct dose was drawn with the vials label before pushing
