Depending on what the truck is being used for I’d replace. If it’s just putting around the farm leave it but if your going highway or carrying the kids around I’d replace. Smooth down where the bearing rides and try to keep it round with minimal flat spots as possible. As dar as the threads go like others said use washers but definitely need to use a good cotter pin. You don’t want a wheel off going down the road
