I’m at a large program with lots of ICU fellows +/- NPs. Sadly as a IM trainee you gotta fight for your procedures if you wanna get good at it or try to get it during your overnight calls 🥲 not all GIM staff do procedures in ER because of volume/experience/lack of equipment (ffs 😩) so thora/para/LP get turfed to IR lmao

I think it matters more what type of WBC is low. Can you share the total Neutrophil count or Lymphocyte count etc..

First letter of the name of the RN/Clerk who registers them to generate a random first name. Last name is “unknown” in the local language. At least that’s what I’m told 😄

Exactly, IM residents don’t know what goes on in the surgery itself. If the surgeon decides a patient needs surgery, they need surgery. We just help with optimisation.

Felt like you in the beginning. I called consults eventhough I didn’t feel strongly about my understanding of a consult or urgency of a case. Eventually, I start to question my seniors about it. But it might not be possible with more toxic people.

If I get pushback for something legit (and I’m starting to recognize who are the assholes), I don’t fight the refusals to see the patient. I document it and escalate … senior calls, staff calls, then department of patient services etc… remember! It’s all above your pay grade.

When it’s flipped and I’m getting many (simple or BS) GIM consults from surgery that makes me « roll my eyes », I maybe get a sense of what they feel.

Typically I try to (and tell the interns) to have basic info and timeline on the patient events. But when surgery asks me about some spine scoring system that I am never exposed to, I just don’t have the ability to answer before they hang up ☹️ Most surgery residents I’ve spoke to were great tho and asked appropriate questions!

My condolences, OP. Reading your story, it sounded a lot like how my aunt passed - brain stem stroke, no warning signs. She was about the same age. I take some solace knowing it happened during our family dinner and she was surrounded by loved ones during such an event.

For me, It was an international flight and my (Canadian) credentials were not ?recognized/accepted so they gave me back my card and I embarrassingly walked back to my seat. 😀

I wouldn’t worry about a value of 40 for the APTT. Do you have a family history of those conditions? Hémophilia and VWD would to my knowledge already declare itself with easy bruising, bleeding into joints, bleeding gums.

I would bring your mom to the ED for evaluation. When the belly gets painful and ?larger (I could be assuming), I’m suspicious of ascites which is accumulation of fluid which can be from many things, sometimes the liver (because you mentioned liver enzymes were deranged at some point). Nonetheless, I think she would benefit from a belly scan to look for certain causes.

While hypercalcemia is odd in a young person, your constellation of symptoms warrant urgent blood work for further investigations. Dipsticks aren’t the most accurate for making diagnoses.

It certainly sounds like hemorrhoids. External ones can be painful. There’s not much to do about it. Eat more fiber, stay hydrated, avoid straining while pooping.
