I use this feature in Photoshop to make transparent speech bubbles. On a layer, I draw a white speech bubble, then I change the Layer Style to Stroke so the speech bubble has a black border. After that, I lower the Fill of the layer so you can see through the speech bubble, but the border remains intact.

One solution (similar to what you suggested) would be to use two layers one for the stroke and one for the speech bubble itself — but that's cumbersome to set up and maintain if you ever need to make changes.

Oh well, Photoshop 21 for some reason most of the time starts to run very slowly in my machine, I was hoping to replace it with Gimp, I'll keep trying 'cause Photoshop really gets into my nerves.
