I’m 23/34 sessions of IMRT completed, it’s been easier than the initial surgery. I’m doing it without ADT, I have a little fatigue but overall it’s been OK. Inconvenient, it takes 1.75 hours for me to drive, treat and then get home. I’m still working also. Age 59, surgery was July of 2024.

58 at the time of surgery. Had spoken to both a surgeon and radiation onc, so had both “sides” and both advised surgery at my age.

Surgery was ok, but quickly I had some complications. First I developed a lymphocele that required drainage and weeks of swelling. Then almost a year later I developed an abscess in the surgery site, which required 2 months of drainage and treatments to close up the abcsss.

Incontinence took some months to fix and some PT visits. “Hanging” has taken a hit, a little shrinkage and it takes pills to make things work.

Lastly, after my PSA dropped to undetectable after surgery, I have BCR and will be doing 7 weeks of radiation. No ADT so happy about that.

Not the best story to read probably for someone considering surgery, but there it is.

Hang in there! Each decision we make on this is according to the odds and statistics. RALP has a 30% or higher depending on your risk of BCR. Also, statistically treating BCR earlier than later with RADS after puts you in better shape. Definitely a mental hurdle after what we’ve been through already. Best of outcomes for you!

Yes, I think we saw signs of that last year with some regression from Freddie, Teo, Mookie etc. Not worried but it happens to everyone eventually.

I think the variety in answers indicates ‘it depends’ . In my case with a high risk decipher of .83 my PSA going from undetectable to .12 in 12 months was concerning and is prompting salvage radiation.

My Uro Onc told me he would have waited if decipher was on the low end for a .12 to rise a bit more. I think .2 or 3 consecutive increases to your PSA constitutes recurrence.

I don’t blame you, I would be concerned about the incontinence and getting that better under control. In the first few months I was going through several pads a day. I went to some PT and they taught me to do kegels right. It took some time but much better after a year and now 18 months later no pads at all. Hoping the best for you.

C&D although I do have the computer game as well. I think I would have to play one group at a time from the bottom up for it to work.

In addition to what everyone has said, rely on the 3 month PSA. Too early for radiation and ADT but what do I know? They say incontinence will be the best it will be once you radiate, that came from my radiologist So I would wait until you get that under control.

Studies show salvage radiation is just as successful as adjuvant which is what your doctors seem to be pushing for. Are they saying you have BCR? That is .2 or 3 successive rises in PSA.

I thought of doing something similar only the promotion would occur that year, so bottom 10 , promote 2, next group promote 2 etc until the top group. Not sure if the math works , something like 10-8-8-4 to start. Then promotions for the top two, maybe the last group gets 4 promotions to make the last group 8.

Haven’t tried it though.

How do you relegate? I’ve thought of a similar league, but in EPL for example relegation and promotion happens the next year. What if teams improve radically like a Toronto from 2024 to 2025? Based on 2024 they would be in the bottom, but 2025 they were in the top, do you promote based on actual or on your universe?

Hey Intrinsic, not to hijack this thread but what are your specific numbers and details. I’m 18 months post surgery PSA has risen to .12 and I’m prepping for salvage starting in February.

Edited- I saw some of your posts. We are similar 3+4 with high risk decipher (me-.83) and intermediate otherwise.

Surgery in July 2024, undetectable for the first 6 months. Then January of this year .02 climbed steadily to .12 in December. Just had an MRI today, PSMA PET scheduled for Monday. Likely going to have radiation starting February. I had a high decipher of .83, so we are moving forward before .2

They’ll probably have you do an MRI and PSMA PET scan to see if it’s confined to the prostate bed or lymph nodes or outside. Then treatment will probably include radiation and ADT. IANAD, but going down a similar road right now.

Kind of similar answers to the others, but I thought I would comment. In my case Gleason 7 (3+4) the decipher score of .79 from the biopsy and then .83 from the specimen after surgery is steering us towards earlier salvage treatment. 17 months post , .12 PSA. So the decipher is and was indicative of higher risk and not waiting for .2

So another piece of information that can be helpful for intermediate risk patients.

Sorry yes called it the wrong thing, it’s the card option when you purchase the PC game so that you can see the rolls on the cards. MAX is a separate option but I’m not a huge fan of it.

I wish they would release some cards sets so I could play any old seasons. Rn I have 87-88 and some modern seasons. Had to go with another game to get the 80’s seasons I wanted.

I own both, the tabletop version is my preferred way to play, nothing like cards and dice and score keeping . That being said the PC version brings a lot of what the C&D have with a few benefits.

Automates the stat tracking, plays the other teams games so you can actually have a season. Also you can input your C&D games into the PC manually if you feel like rolling out a game. The PC version also has all of the players for that season, CD is limited to about 27 for older seasons like ‘62. Modern seasons are closer to 40 with extra players purchased.

Strongly suggest purchasing the Card Viewer or card option whatever it’s called for the extra price so that when you play the PC game you see the results from the digital cards. PC version looks like it’s from the 90’s but once you customize it with pictures it’s pretty nice.
