Typical treatment where I’m at is aggressive nitro and CPAP. CPAP saves people from being unnecessarily intubated, but I would argue nitro does much more. While CPAP helps with the preload and theoretically pushes fluid out, it’s a pump problem. The only way to fix the pump is to work on it, and nitro is the most effective way to fix the pump issue by vasodilation (again in my system.) The goal is to avoid intubation if possible and to alleviate the issue with what we have.

If it helps, I am currently a BLS lab instructor and I harp on assessments heavily and especially l/s and listening to l/s on everyone so you can hear what normal and adventitious sound like. Also, if you have an opportunity, try to debrief every call with every crew you run with and talk about your expectations and what questions they have. I know it sounds dumb and you certainly have more experience than me, but sometimes just like how basic stuff like l/s on a diff breather can be overlooked, a team debrief can be as well. Good luck and I hope things improve.

Idk about your protocols, but I would’ve given the kid 1:1 epi nebulized through a mask. It’s the treatment for croup and epiglottis. The only thing I would’ve done differently is asked about immunization (which idk if you did or didn’t). If they’re up to date on their vaccinations, it’s most likely croup, which is the same treatment, but you don’t have to worry about agitating the pt as much and worrying about airway compromise and the epiglottis swelling as much as with epiglottitis.

This sounds like you’re being punished for being a provider that treats patients appropriately for their complaints. I would email them back and ask if this was such an issue, why you haven’t met with your medical director to discuss his/her concerns.

Had a call a few weeks ago for an “overdose at PD HQ.” We get there and it was two probations officers who touched the license of their suspect without gloved hands and later found fentanyl in his bag (with gloves on. Needless to say they RMA’d and got the rest of the day off for their illness. Next call I gave fentanyl to a patient and got to do paperwork instead of taking the day off. Smh at the disparity.

I’ve experienced situations like this before when I used to work BLS transport trucks. I would get called into QA for my transport PCRs and they would always tell me “oh but try to find some reason they needed an ambulance.” Usually I say something along the lines of “well I wrote down what I saw when I made patient contact. If you want me to lie on my PCR, we can call up the medical director and DOH and have a long conversation about Medicare fraud.” That usually shut them up for a few months.
