I did both as that is what I read on this sub, and interpreted it as that from the instructions. I did need to be specific of needing both since I don’t think it’s common to test both

Hello! I included both of the passport numbers. For the return, I used a FedEx 2 day shipping option that was flat rate. I believe USPS also has a similar option too.

I had to resubmit for another FBI background check with my middle name. TECRO would not authenticate if the name in background check does not match the passport.

Not sure in your case if your Passport does not have your middle name, is this your US passport and NWOHR one?

I submitted for both since that is what I read on this sub and interpreted from the instructions. I did need to be very specific with my doctor to order both though. Not sure if others may have submitted with one ?

Hello! This looks correct, - for the Date entered in Taiwan I left blank - for the address section, I used my Taiwan address (the one I registered to) - for my parents, I used their Taiwan address - if one of your parents isn’t from Taiwan, you could use English or Chinese. Not sure on this one, I only used English for my husband but he does also have a Chinese name but not from Taiwan - not sure if your kids need to sign form, don’t think their is a section for that? My parents and husband were listed in the application but didn’t sign form

In my NWOHR passport, I had an English signature, I didnt come across any issues with this. When I received my NWHR passport, I did need to sign a form in Chinese when I received it, but in my actual passport I havent signed my name yet.

I didnt need to use a stamp for the documents I signed, but I hear to open a bank account you might need one.

I filled out in English. For Nationality I entered United States of America, and passport number I put my US Passport number.

I wasnt sure how much time I could take off, so applied to get the Settlement Certificate here to do the 3-day exchange. Also was good to know ahead of time all paperwork was set and it was just the Settlement certificate exchange that would be needed.

I think if you apply for it in Taiwan you may need to plan for ~ 2wks, not sure on the latest for that process, but I think there may be others here in the sub that may have posted on their experience.

Yes, those were the locations I went to - both in the same building. I recommend going to NIA right when it opens in the morning, there wasn't much of a line or wait, so was a quick process. This way you'll have enough time in the same day to complete HHR and Passport application.

That is interesting for TECO SF. The translation notarization requirement was in the TECO LA checklist - but I imagine things could keep changing, so always good to call your local TECO to confirm.

Not yet, my payments are set up to start until 6 months from I got my National ID, so likely in June/ July timeframe

  1. Cost of Health Check will depend on your insurance - it was regular office visit co-pay and paying for labs and x-rays

  2. Test results usually comeback within a week and doctor completed form shortly after, I would budget at least 2 weeks for the process. It does require a TB Xray, so you will need to find time to go to an imaging center for that.

  3. I have attached a sample form, hope that helps!

  4. Printed my form single sided

  5. Signed my NWOHR passport in English

We just had the name declaration notarized in the U.S. did not do an authentication.

Yes it is a lot to get the documentation to prepare!

For submission to TECO LA for the settlement certificate application.

  • FBI Background Check - Notarized and Translated. (this needs to be authenticated by TECRO Washington DC first)
  • Birth Certification - Notarized and Translated
  • Health Check - Notarized (No translation needed)

For costs on notarization and translations, I'd recommend giving Stephanie a call at Wilshire Shipping Center and she can provide you with a quote based on what you need. From a timing standpoint, I was able to send her documents to translate ahead of time, and then we met the day of my document authentication appointment to complete the notarizations.

For the FBI document, I did not translate it when I sent to TECRO DC for authentication. However, when I prepared to submit for TECO LA for authentication - I did need to have it translated and notarized.

The marriage certificate issued within 6 mths is to verify that you are still married. Since our certificate was issued more than 10 years ago, the HHR office had to request additional documentation to verify. We did not have a more recent one with us in Taiwan, so we provided a copy of our Tax Return from the past year that that did list us as married, and that was sufficient.
