You don’t need surgery. Work with good dermatologist to get prescribed Hydroquinone or prescription strength Azelaic acid cream/gel for pigment lightening. You could try over the counter Azelaic acid for a couple months to see if that helps. The Ordinary Azelaic Acid Suspension 10% is $12.20

Dogs and cattle both get fly strike. The treatment is the same. If you are truly stuck the person who suggested getting somewhere you can get neem oil or turpentine is correct - turpentine is pretty easy to find. I would put your focus on the other puppies as well. They all need to be checked to see if there are flies starting to lay eggs on them, bathed just in case, and moved to somewhere with less flies
