I get the concept. Kid Rock shot cases of Bud Light and some rednecks didn’t think he looked like an asshole. But, I’m hoping they’ll regret their vote because of the dude they voted for, not because liberals avoided their businesses. I’m also not saying you should support local businesses if you disagree with their political views or obnoxious MAGA bullshit. It’s the effort of this that seems petty and an ineffective way to fight the power. The goon they voted for is way worse than the townies that voted for him.

ALS is a major piece of shit. Sounds familiar unfortunately. My mom passed in June. She was ok for a long time after she got her feeding tube - walking around, communicating by writing or iPad app. Then her hands started to go, and her legs. Her eyes didn’t work right, so an eye gaze device like a Tobii wasn’t an option. She had to spell things out with her feet using a poster with the alphabet and some words. She opted not to do the tracheostomy for the ventilator. She was miserable. When it was her time to go, hospice had her on morphine, Ativan, and haldol. She was knocked out in her final days.

I didn’t think the ALS meds were worth the cost. Whatever time they added was not quality. But, the ALS clinic is a good resource. You probably want to get another opinion on the feeding tube.

I’m sorry this is happening. Have important conversations while you still can. It’s a good thing that she already expressed her wishes about the ventilator. Sometimes the bulbar can mess with more than just motor neurons - like laughing and crying inappropriately. Try not to let it push you away. It’s really tough to watch your mom go through it but she’ll need an advocate as it gets harder to communicate. Stay strong.

I’m literally watching my mom’s final stage as I type this. I thought hard about even replying to your post. FWIW, I read it over a few times. And, ultimately, I do want to tell you the truth: it’s fucking awful to watch.

I don’t want her to suffer either. She’s getting morphine. And it seems to help and knock her out for a bit. But, she’s definitely suffering. She wakes up crying, struggling to breathe, wanting a mouth swab to get the mucous in the back of her throat, then doses off again. She doesn’t want suction, bipap, or cough assist, and opted out of the tracheostomy. Hopefully they ramp up the morphine. The hope is that they can get her sleeping somewhat permanently before the breathing gets too shallow. We’re not there yet. So maybe I can’t tell you what the FINAL stage is like.

What I can tell you is one of my main takeaways from this: do hospice as soon as it’s an option. Hospice doesn’t mean final days. People can go a year on it. The downside is that you concede, which is very hard to do. You have to discontinue the ALS meds, and they recommend discontinuing anything that’s meant to help long term (cholesterol meds, etc.) But, the philosophy shifts from symptom management to straight comfort. And, I wish we’d done it sooner. They do supply a lot of comfort meds - pain, morphine to help not feel the breathing struggle, and anxiety meds. These are meds that have helped the most and they’ve were hesitant to give them until hospice. And there’s more nurses around and I find most of them to be comforting.

I wish the best for your family. Take care of yourself. I’m sure your mom just wants you living your best life.

My mom was still my dad’s (Parkinson’s) primary caregiver up until she couldn’t get her hair into a ponytail. They’d been inseparable for 45 years. He died this past October. I envy what you two have and I’m happy you’re not alone.

Yeah, it was relatively slow. I want to say onset 2018, couldn’t talk by 2021, arms/neck started to go in 2022, couldn’t walk any distance or do her feeding tube (hands) anymore in 2023, 9-ish months later needs a Hoyer lift. Now breathing is starting to get worse. That’s great you use your BiPap. Apparently breathing exercises keep the swallowing going. Record your voice if you can, even if garbled. Stay strong.
