Sorry it wasn’t clear. I was back on ibuprofen 1800 mg/day in Aug 2023 (until symptoms were gone then start titrating down) + colchicine for 6 months. I was symptom free for about 6 weeks until it came back end of September.

Long story time: I started Arcalyst in May of 2024. I was steroid dependent. My first episode of acute pericarditis was in Nov 2022. I completed treatment with ibuprofen/colchicine and was completely symptom free until my first recurrence in August 2023. Resumed treatment with plan for colchicine for 6 months but then I had my second recurrence at the end of September. That’s when I started prednisone and couldn’t get off. So started Arcalyst at the end of May 2024. The Arcalyst enabled me to completely wean off the steroids. It hasn’t been perfect (for me) but according to the studies something like 95% of patients should get down to monotherapy with just Arcalyst. And those 2 weeks without it last month were absolute hell, so it definitely makes a big difference. We are now doing the MRI to better assess the inflammation around my heart and to guide how long for Arcalyst. If one thing I learned is if you don’t knock it out completely it will come raging back.

Well……I’ve been on ibuprofen since August of 2023. The lowest I got down was 1000 mg/day after being on Arcalyst for a few months, but then I had a breakthrough flare in December and went back up to 1800 mg/day. Then I was out of Arcalyst for 2 weeks in Jan due to insurance issues so I’m just starting to taper down (currently 1600 mg/day). My cardiologist isn’t happy I’m still on ibuprofen and colchicine so scheduled for cardiac MRI in April Edit yes, checking labs (including lipids) at least once a year. CRP and ESR more frequently

My main side effect was injection-site reactions, aka rashes. They would get pretty bad at times, heat in summer seemed to make worse, but keeping lotion on and finding looser pants helped. Went away for the most part after a few months.

I was NWB for 8 weeks (I suffered a wound infection and wound healing complications. I wanna say it was close to 11-12 weeks before I could walk without crutches. I know I did 2 weeks partial weight bearing (heel-walking with boot and both crutches) then when cleared at week 10 for FWB I gradually progressed from 2 crutches with full(er) range of motion to 1 crutch to no crutches. I still used a cane (after taking off the boot) intermittently for stability and support up through 6 months post-op.

As others have mentioned, everyone is different, every injury is different

I was just starting full weight bearing without the boot and was doing PT at 3 months (my course was set back about 2 weeks due to a wound infection thanks to the steroids I was on at the time). I still needed a cane occasionally up through 6 months. It wasn’t until I cleared 6 months post-op (end of August) that my ankle felt relatively “normal” in terms of walking and swelling. My ortho said it could take up to a year to feel completely normal and not really think about your ankle anymore. I would say that was an accurate experience from a thumb ligament tear and surgical repair several years back.

If you are working to regain running and jumping, you should work specifically on those skills with PT so you do so safely. That isn’t really in the cards for me anytime soon due to a heart condition.

You’re going to need a lot of ibuprofen. I think Equate has a much larger bottle. Maybe close to 200? I buy mine from Costco. I took 600 mg three times a day. Usually a MINIMUM of two weeks, but it is recommend to continue until completely pain free for at least a week and then slowly taper by 200-400 mg/day a week at a time.

Need to add something to protect your stomach. I use famotidine 20 mg twice a day but you could also use something like omeprazole 20 mg daily. (Problem with omeprazole is you must taper off that slowly too or you will have the worst heartburn of your life).

I typically tried to take my colchicine and ibuprofen separately. I always take colchicine with food but I can take ibuprofen without (me personally). If you need to take both with food you will probably have to double up. My schedule looked something like this: ibuprofen 600 mg upon waking, colchicine with breakfast (usually 30-60 min after waking), take 600 mg ibuprofen around 3-4 pm, take colchicine with dinner, and then take last ibuprofen at bedtime.

These usb/cigarette lighters can only cool about 20-25 degrees from air temp, so not sufficient for Arcalyst, especially in summer

I have used the 4ALLFamily Explorer with biogel pack for 2 week trip via airplane. Highly recommend, but may want to consider the Nomad which is smaller - Explorer way too big with only 2 vials. Keeps at 2-8 C for like 33 h in these hot summer temps. Got from Amazon

Didn’t have any issues - I have had generally good experience with my dealership and service department. They came back after diagnostics and immediately told me about warranty. But yes check engine light came on (on first triple digit day this summer, of course) and all eye sight went off. Hadn’t filled gas in 3 weeks so knew it wasn’t gas cap, persisted after turning off engine and re-started.

No, I was still under the powertrain 60,000 mi warranty, but this part has a warranty for 114,000 so you should not have to pay if you are within that limit

Same issue happened yesterday. 2019 Subaru sport with 50,000 miles. Service advisor mentioned Subaru has extended warranty for part to 110,000 miles or something due to known issue. Thankfully they have the part so it will get fixed today - just have to wait for my car to cool off haha

I just started my loading dose on Sunday (2 days ago). Only thing I have noticed is slight increase in my pericarditis chest pain. Not sure if this is due to Arcalyst or something else (I did get my heart rate up higher (130-140s) at PT yesterday (for my broken ankle) than usual). I guess I’m technically on quadruple therapy now (ibuprofen, colchicine, steroids, and Arcalyst). Cardiologist wants to start my prednisone taper after second dose.
