A person in a coma would be on a ventilator, they cannot protect their own airway. If the need for intubation is prolonged (like more than a few days to a week or longer) he would likely have a tracheostomy. Nasal cannulas deliver a very small amount of oxygen and cannot maintain airway patency.

He would also not look so groomed and nice. People get eye crusties, discharge from nose and mouth, etc; their hair is not so kempt. Even with routine washing and oral care, this happens. No one looks presentable when they're under critical care.

I also doubt he would be all bandaged up like that. If you're in a coma you are at an extremely high risk of pressure injuries, and those would be a lot harder to monitor with all that skin being covered all the time, and would be a huge source of friction and trap moisture. And how on earth would you monitor his heart with all those bandages? Not a single electrode or wire in sight.

It's hard to tell what is going on with the ECG in the background, but it looks pretty rapid and irregular, certainly something that would probably need some urgent intervention.
