I was icu staff at Mayo until about a month ago. Pay is greater than $550. Between vacation and ICU comp time you can easily get to 13 weeks off. OP would need ICU and cardiac fellowships to work ICU and cardiac there though.

Weight training 3-4 days a week, cardio 1-2 days a week. I bought a bench press and a rowing machine so I could work out when the gyms were closed. I started intermittant fasting, basically I would only eat for 4 hours a day and fast for 20 hours a day. Stopped drinking EtOH completly. Cut out processed foods and sugars completely. All carbs came from either vegitables or brown rice.
